| Objective To investigate the correlated factors of atherosclerosis in nonalcoholic fattyliver disease.Methods A retrospective case-control study was performed to compare some clinicaldatas of 397 hospitalized patients in Peking University Shenzhen Hospital from January 2004 toMarch 2010 who were examined by color Doppler sonography, and they were divided into groupA (NAFLD with higher mean IMT), group B(NAFLD without higher mean IMT). Another 154hospitalized patients were check-up as contrast group that was named group C(higher mean IMTwithout NAFLD). Their sex, age, blood pressure, body mass index, fasting plasma glucose,fasting insulins, HbAlc, lipids, enzymes, uric acid, C-reactive protein, Homeostasis ModelAssessment of Insulin Resistance index(H0MA-IR) and carotid Intima-media thickness(IMT)were measured and analyzed.Result1. The case group have 253 subjects with higher mean in IMT, the rate was 63.7%, with39.0% in men and 24.7% in women. The other case group have 144 subjects without highermean in IMT, the rate was 36.3%, with 20.7% in men and 15.6% in women. The contract grouphave 154 subjects who were not NAFLD with higher mean in IMT, the rate was 63.0% in menand 37.0% in women.2. The levels of BMI,SBP,FPG,FINS,HbAlc,HOMA-IR,TG,ALT,γ-GT,CRP,IMT were increased. There were significant differences in A and B groups(P< 0.05). Specially some levels of BMI,HbAlc,HOMA-IR,CRP in the subjects who suffer NAFLD with thechanges of IMT were increased comparing with NAFLD without higer mean IMT(P<0.01).There were significant differences in BMI,HbAlc,HOMA-IR,TG,γ-GT,CRP,IMT in Band C groups(P< 0.05). There showed no significant differences in sex,age,DBP,TC,LDL-C,HDL-C,UA in three groups(P> 0.05).3. Multiple Logistic regression analysis showed BMI,HbA1c,TG and CRP had the statisticdifferences and regression coefficients were 0.440, 1.242, 1.822 and 0.778, OR were 1.961,1.785, 2.022 and 1.862.4. Multiple stepwise regression analysis demonstrated that NAFLD with the changes ofIMT was correlated with age, TG, FINS, HbAlc and CRP, regression coefficients were 0.100,0.098, 0.159, 0.101 and 0.524.Conclusion: Insulin resistance and obesity may be interlock the mechanism of thenonalcoholic fatty liver and carotid intima-media thickness. The correlated risk factors of thenonalcoholic fatty were obesity, hyperlipemia, higher mean of HbAlc, subclinical inflammatoryfactor. Increased Carotid IMT and subclinical inflammatory factor is observed with NAFLD, andthe risk of developing atherosclerosis is increased.... |
