Objective: To investigate the change in neuromuscular blocking effect of cisatracurium in patients with obstructive jaundice .Methods: forty patients undergoing elective surgery under general anesthesia were randomly divided into 2 groups: (Ⅰ)hepatosis group including 20 ASAⅢpatients with obstructive jaundice and (Ⅱ)control group including 20 ASAⅠ-Ⅱpatients with normal liver function.All patients received a combined anesthesia with propofol, isoflurane and remifentanil.The 3×ED95 (0.15mg/kg) of cisatracurium was given during anesthesia induction. Neuromuscular blockade was assessed with TOF-Watch acceleromety [Organon,Holland] using single twitch stimulation at the adductor pollicis muscles during operation. When the value of single twitch and control (T/Tc) reached to 10% , 1.5×ED95 (0.075mg/kg) of cisatracurium was added.Results: There was not statistical significance in the onset time , the time to 90% block and clinical duration between the hepatosis group and the control group.Conclusion: The pharmacokinetics of cisatracurium in patients with liver dysfunction induced by obstructive jaundice did not change obviously compared with patients with normal liver function.
