Along with the develope of meditech, the death of premature is descending to some extend. The reasons of premature birth are complicated and inevitable, that premature birth rate and death rate is fairly high, and the death rate is corralated with Gestation Age and Birth Weight. The objectives of this paper is to study the relationship and differences in newborn premature infant with different conceptus age by comparing the difference between Ghrelin level, Insulin level and birthweight, to find new index of premature death assessment.Object and Method1. Object select 48 premature infants dwell on department of neonatus in ShanXi ch ildren's hospitai, from 2008-08-01 to 2009-05-31. Selected standard:gestational age<37 we-eks,appropriate for gestational age infant(AGA), admission age<24h, except pathoglyce mia, ARDS,scleredema,parentetal alimentation,injection,polyembryony,chromosomal aberratio n and passage Metabolic disease. Except children whose mother with gestation period hy pertention, endocrine disease, severe anaemia,malnutrition,infection,uterine abnormalities,ute rine fibroids,abnomaly of umbilical cord and placenta.2. Method get femoral vein blood 2 ml to put in a tube promoting coagulation whe n the infants addmited to hospital. Centrifugate 1000r/10min, put the supernatant fluid int o refrigerator under-70℃.Mensurate Ghrelin with ELISA, Insulin with raddio-immunity method.3. Statitic method treat the date with SPSS 13.0 statistic software, represent date with Mean±SD. Analyze the differences between Ghrelin level, Insulin level, gestation age and birthweight with univariate ANOVA. Use pearsonn linear correlation to identify the relationship between them.Results1. generral conditions:Gestational Age (35.01±0.162) weeks, Birth Weight 1605g-3200g (2400.8±65.88) g, Admission Age (6.3±0.7) h, obtain blood time (9.3±0.6) h.2. Ghrelin level decreased remarkably in bigger conceptus age premature infant as compared with that of smaller conceptus age ones (p<0.05); Ghrelin level decreased in bigger birth weight (BW)premature infant as compared with that of smaller BW ones (p<0.05), Ghrelin level had negative correlation with birthweight, conceptus age and Insulin level (r=-0.692,-0.505,-0.692, P<0.05). while Insulin level is in opposite position(r=0.794,0.825, P<0.05).ConclutionThe develope of fetus depend on nutritive material,beside the controls of various hormone and bioactive substance. This paper show that Ghrelin level decreased in bigger birth weight (BW)premature infant as compared with that of smaller BW ones, Ghrelin level had negative correlation with birthweight, conceptus age and Insulin level. While Insulin level is in opposite position, indicate:Ghrelin can promote the release of growth hormone(GH), encourage development of fetus, weight rising; meanwhile bigger weight restrict the release of growth hormone(GH), so fetal weight can be seen as a negative feedback signal the express of Ghrelin, but the concrete mechanism is not yet clearly. Ghrelin and Insulin have negative relationship, though the interaction between them may conduce to development and growth of fetus, it does not make to overgrowth or limitation. Ghrelin may take part in inter-palace development and axial metabolism with GH in form of degeneration. Many studies conclude that:Ghrelin participate metabolism of carbohydrate, fat, and energy, is an importent factor of fetal growth. Ghrelin may provide significant index in neonatal death risk assessment.
