Objective:To investigate the aldehyde dehydrogenase 2 (ALDH2) level in patients with acute myocardial infarction (AMI).Method:42 patients were enrolled in this study between Feb 2009 and Jan 2010. The genotypes of ALDH2 were determined by ALDH2 genotyping kit (gene chip) . The genotypes of all cases were the GG genotypes. The patients were divided into AMI group(27) and control group(15). Level of ALDH2 activity was determined at 12 hours, 24hours and 48 hours after typical angina individualy. Their levels were compared with those of the control group individualy, which comprised 15 healthy volunteers. The AMI group was divided into two subgroups according to the final infarct area: large infarct area subgroup(>15%) and small infarct area subgroup (≤15%). According to heart function, the AMI group was divided into three subgroups: KillipⅠsubgroup, KillipⅡsubgroup and KillipⅢ-Ⅳsubgroups. The level of ALDH2 among different subgroups were compared. Final infarct area of the AMI was assessed by the QRS score system of Selvester. ALDH2 activity was measured by the method of acetaldehyde metabolism. All data were collected and analyzed with SPSS v.17.0 software.Result:1. The ALDH2 level at 12 hours, 24hours and 48 hours in AMI group were all significantly higher . The ALDH2 level descended with the lapse of time, and reach the highest point at 24 hours after typical angina. 2. The total ALDH2 level of the small infarct area subgroup were higher than that of the larger infarct area subgroup(p < 0.01). Linear correlation analysis showed that the total ALDH2 level was negatively correlated with infarct area(r= -0.484, p=0.01).3. The total level of ALDH2 in KillipⅡsubgroup and KillipⅢ-Ⅳsubgroup were lower than that of KillipⅠsubgroup significantly.Conclusion:1. The level of plasma ALDH2 activity was dynamic changes in patients(GG genotype) with AMI, reached the highest point 24 hours after severe chest pain.2 Changes of plasma ALDH2 activity were related to severity of infarct size. The level of plasma ALDH2 activity were also correlated with heart function. These results suggest that ALDH2 could possibly inhibit myocardial ischemic necrosis of the patients with AMI.
