| Objectives:To count all of the 2066 cases who chased for the treatment of the vulvar discomfort or the abnormal vaginal discharge and the abnormal examination of vaginal discharge in our hospital clinic (Second Xiangya Hospital, Central South University) from November 2009 to January 2010. Then to analyze the characteristics of vaginal micro-ecological environment balance disorders and determ the detection rate and the constituent ratio, age distribution of various vaginal inflammation and various factors associated with bacterial vaginosis, vulvovaginal candidiasis. To guide clinicians for the prevention and treatment of vaginal microenvironment balance disorders.Methods:Collect the information of 2066 cases with abnormal vaginal discharge or genital discomfort and the abnormal examination of vaginal discharge from November 2009 to January 2010 report of the Second Xiangya Hospital gynecology clinic, this study were to investigate the imbalance of the distribution of the vaginal microenvironment in the form of a questionnaire. Then abstract all of the child-bearing age bacterial vaginosis (152 cases) as the BV group and all the child-bearing age vulvovaginal candidiasis cases (585 cases) as the VVC group.And the marched group is composed by 150 cases with normal vaginal microbial distribution and no syptom and sign. Performed the case-control analysis.And obtain the comparative analysis through the single factor and multi-factor non-conditional Logistic regression analysis, OR and 95% CI.Then draw bacteria vaginosis and vulvovaginal candidiasis in the relevant factors and the risk factors. It is important prevention and treatment of the disease.Results:(1) The abnormal vaginal microbial detection rate was 51.33%, the vulvovaginal candidiasis'constituent ratio was 28.99%, the bacterial vaginosis'constituent ratio was 8.67%, the trichomonas vaginitis' constituent ratio was 2.04% and mixed vaginitis'was 0.48%, and other undefined ones'was 59.92%.(2) In all of the age group's constituent ratio of the vaginal microflora balance disorders:-20 age group was 2.4%; 21-30 age group was 32.7%,31-40 age group was 38.2%,41-50 age group was 21.7%,> 50 years age group was 5.0%.Except the distribution of the median age of the mixed vaginitis was in the 21-30 age group, the distribution of the rest kinds of vaginal inflammation in the median age was in the 31-40 age group.(3) The bacterial vaginosis related factors:occupation, education, menstrual cycle, vaginal washing frequency, IUD, sex nearly 7 days, frequency of sex per week in single factor non-conditional Logistic regression analysis, P<0.05, However, in multi-factor non-conditional Logistic regression analysis, just menstrual cycle, vaginal washing frequency and IUD, P<0.05, owns the statistically significant difference.(4) The vulvovaginal candidiasis related factors:education, menstrual cycle, vaginal washing frequency, use of antibiotics in the past 2 weeks, use estrogen and progesterone drugs in the past three months in single factor non-conditional Logistic regression analysis, P<0.05, However, in multi-factor non-conditional Logistic regression analysis just vaginal washing frequency and use of antibiotics in the past 2 weeks, P <0.05, owns the statistically significant difference.Conclusions:(1)In the vaginal microflora balance disorders, the constituent ratio of vulvovaginal candidiasis was higher than that of bacterial vaginosis, trichomonas vaginitis and mixed vaginitis.(2) Vulvovaginal candidiasis, bacterial vaginosis, trichomonas vaginitis and mixed vaginitis were most occurred in the reproductive-aged females.(3) The vaginal washing,the later stage of the menstrual cycle and the IUD are the risk factors of the bacterial vaginosis.(4) The vaginal douching and the recent use of antibiotics are the risk factors of the vulvovaginal candidiasis. (5) Beside the BV,VVC,TV and the mixed vaginosis,there are some kinds of undefined vaginal microflora balance disorders need be defined. |
