Pathologic And Immunologic Change In Nonalcoholic Fatty Liver Disease With Or Without Subclinical Hepatitis B Virus Infection

Objective The aim of this study was to explore the pathologic and immunologic in nonalcoholic fatty liver disease(NAFLD) with or without subclinical hepatitis B virus infection,and to detect the differences between clinical and pathologic features,the expression of the immune index on hepatic tissue and the influence of subclinical hepatitis B virus infection.Methods 47 patients was divided into two groups:group A:33 patients with NAFLD;group B: 14 patients with NAFLD and subclinical hepatitis B virus infection. Their height, weight and blood pressure were measured. To all the subjects diagnosed by B-mode ultrasonography and liver biopsy,level of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),total cholesterol(TC),triglyceride(TG),γ-glutamyl transpeptadase(GGT),HBsAg,HBV DNA were measured.The degree of steatosis,inflammation,fibrosis and the expression of IgA,IgM,IgG,C3d,C3c,C1q,CD20,CD45RO on liver tissue were observed.Results 1,There were 29 males and 4 females in group A with a mean age of 36.8±10.8 years,while there were 11 males and 3 females in group B with a mean age of 40.7±10.9 years.There was no significance differences between the two groups.The diagnostic rate of NAFLD by B-mode ultrasonography was 74.5%,while 57.4% was conformity to the histologic study.2,There was no significance differences of serum ALT,AST,GGT,TG,TC between the two groups(P >0.05)。.3,There was no significant correlation between serum ALT,AST,GGT,TG,TC,BMI and the degree of steatosis ,inflammation , fibrosis(P >0.05).4,The expression of IgA,IgM,IgG,C3d,C3c,C1q,CD20,CD45RO in liver tissue had no significantly differences between the two groups(P >0.05). The deposition of IgA,IgM,IgG in liver tissue was observed.The teatosis degree of the livers was positively related to the deposition of IgA,IgM,IgG in liver tissue(P <0.01). The expression of C3d,C3c,C1q,CD20,CD45RO was not significantly related to the degree of steatosis , inflammation and fibrosis(P >0.05).Conclusion 1,The clinical index and the diagnosis of B-mode ultrasonography did not conformity to pathologic process in NAFLD patients. Liver biopsy is the essential way to make a correct diagnosis of NAFLD.2,The deposition of IgA,IgM,IgG in liver tissue in NAFLD patients was significantly related to the degree of steatosis. The underlying mechanism was unclear. These suggest that subclinical hepatitis B virus infection has no clinical or pathologic influence on NAFLD.
