Colorectal cancer is one of common malignant tumors. The treatment is the multimodality therapy (including surgery, radiotherapy, chemotherapy, biological therapy and traditional Chinese medicine). With the development of medical technology, the 5-year survival rate and quality of life have significantly improved, but the clinical effect is still unsatisfactory. The important cause with impacted on the treatment long-term survival rate of patients with colorectal cancer is mainly the proliferation of cancer cells, that is, colorectal cancer recurrence and metastasis, particularly liver metastasis. Therefore, the effective prevention method of recurrence and metastasis with colorectal cancer is needed urgently to find. The occurrence and development of tumor is the result of multi-step and combination factors. The body's immune function plays an important role on the occurrence, progress and treatment of colorectal cancer. The Traditional Chinese Medicine in improving the body's immune system has its obvious advantages. Many reports suggested that Traditional Chinese Medicine has wide foreground in antitumor.OBJECTIVE:1. To explore the effects of fluorouracil implants on immune function in patients with colorectal cancer and to observe the safety of fluorouracil implants.2. To observe the effects of lentinan combined with fluorouracil implants on perioperative immune function and the quality life in patients with colorectal cancer.METHODS:From March 2008 to November 2009,132 hospitalized patients with colorectal cancer were diagnosised by preoperative colonoscopy and postoperative pathological in this study.132 cases with colorectal cancer were diveded randomly into radical operation for colorectal cancer (50 cases), fluorouracil implants group (42 cases) and lentinan combined with fluorourcail implants goup (40 cases). The value of NK cell, T cells, blood cells,liver function and renal function were detected on the different time, including preoperation and 4,7days after operation. The patient's Karnofsky scores, Qi symptom score, postoperative intestinal function recovery time, and the abdominal complications were observed.RESULTS:1. The T cells and NK cells comparison between fluorouracil implants group (treatment group) and radical operation for colorectal cancer (control group):No significant differences of NK, CD3+, CD4+, CD8+, CD4+/CD8+ were detected between the two groups at preoperative (P> 0.05); at 4 days postoperation, treatment group NK, CD4+/CD8+ significantly decreased compared with the control group, CD8+ was significantly higher compared with the control group (P<0.05); at 7 days postoperation, treatment group NK, CD3+, CD4+, CD4+/CD8+ significantly decreased compared with the control group, CD8+ was significantly higher compared with the control group (P<0.05); abdominal complications between the two groups was no significant difference (P>0.05). The white blood cell counting between the two groups was a significant difference (P<0.05), PLT, HBG were no significant difference between the two groups at preoperation and 4,7days postoperation (P>0.05); there was a significant difference on liver and kidney function at 7 days postoperation between the two groups(P<0.05). There was no significant difference on Karnofsky score and Qi-deficiency syndrome Points between the two groups (P>0.05).2. The T cells and NK cells comparison between fluorouracil implants group (control group,42 cases) and lentinan combined with fluorourcail implants goup (treatment group,40 cases). No significant differences of NK, CD3+,CD4+, CD8+,CD4+/CD8+ were detected betwwen the two groups at preoperative (P> 0.05); at 4 days postoperation, CD4+, CD4+/CD8+ was significantly higher than the control group, CD8+ was significantly decreased (P<0.05); at 4 days postoperation, treatment group CD3+, NK, CD4+, CD4+/CD8+ was significantly higher than the control group, CD8+ was significantly down,(P<0.05); intestinal function recovery time was no significant differences (P> 0.05); there was a significant difference on the Karnofsky score and Qi-deficiency syndrome Points between the two groups (P<0.05).CONCLUSION:1. Intra-abdominal implantation of fluorouracil implants during radical surgery of colorectal cancer is safe and feasible, which does not increase the rate of postoperative complications. However, there is a little influence on immune function in patients with colorectal cancer.2. Lentinan combined with fluorouracil implants can improve immune function and enhance quality of life with in patients with colorectal cancer.
