Objective: To investigate the value of pro-BNP in the differential diagnosis of dyspnea caused by heart disease or pulmonary disease in order to provide clinical diagnostic information. Methods: Choice 124 patients with dyspnea symptom from December 2008 to December 2009.The pro-BNP of patients suffered from dyspnea which caused by heart failure or pulmonary disease were measured by radioimmunoassay and compared the results. There were 124 subjects, including 74 patients suffered dyspnea which caused by heart failure and 50 patients suffered dyspnea which caused by pulmonary disease. All of them underwent right heart function test, pulmonary artery pressure (PAP) and blood gas analysis. Compare the plasma BNP level in the two groups with ejection fraction, PAP and PaO2. Results:1.The pro-BNP level of the patients who dyspnea were caused by heart disease 2390.39±600.62 pg/mL were obviously higher than those of patients whose dyspnea were caused by pulmonary disease 965.40±487.22 pg/mL (P <0. 05). 2. The plasma BNP level was positively correlated with ejection fraction, PAP and PaO2, and it was negatively correlated with ejection fraction and PaO2.Conclusion: 1.pro-BNP, which has remarkable clinical significance, can be used as the indicator in the differential diagnosis of dyspnea.2. In the two groups, PaO2 was negatively correlated with the plasma pro-BNP. 3. In the patients suffered dyspnea which caused by heart failure find that the lower right heart function, the higher level of the plasma pro-BNP. 4. In the patients suffered dyspnea which caused by pulmonary disease find that the higher pulmonary artery pressure, the higher level of the plasma pro-BNP.
