The purpose of the study was to determine the effect of blood glucose on metabolism in type 2 diabetes and assess the nutritional status of patient.The patients were lived in endocrine department during 2008.12~2009.12, they had similar pathogenetic condition and diagnostic criteria. They were selected according to diagnostic criteria of WHO in 1999. In accordance with FPG the patients was randomly divided into 5 groups:A(control group),B (FPG<7.0mmol/L (126 mg/dl)),C7.0mmol/L(126 mg/dl)=FPG<8.0mmol/L(144mg/dl), D (8.0mmol/L(144 mg/dl)=FPG<10.1mmol/L(182mg/dl)),E (FPG=10.1mmol/L (182mg/d1).Every group has 30 members. The FPG, blood fat, plasma protein and microelement,24 hour urine nitrogen, resting energy expenditure, respiratory quotient, non-respiratory quotient, substrate utilization and somatometric measurement were determined and compared each other.1 The basic data The difference of height, weight, BMI and sex had no statistics meaning (P>0.05).(1) Resting energy expenditure The analysis shows that it differs a lot in different groups (P=0.023).There was a significant difference on the REE and predictive value. And multivariate regression analysis indicated that FPG is a factor of REE.①RQ and npRQ were measurement from all cases, and the differences were not founded (P>0.05).②The rates of protein utilization in groups were significant (P=0.041). But there were no statistically difference on the utilization of fat and carbohydrate(P>0.05). By statistics, no-significant difference was revealed between the levels of urinary nitrogen(P>0.05). The serum makers of TG,CHO,HDL,LDL had significant differences between every groups. There were significant differences in TG and CHO among the groups of B,D and C (P<0.05),and the HDL significantly differed between the groups of D and A (P=0.023). The differences of LDL was found among the groups of B,D and A(P<0.05).The differences of plasma protein(TP,ALB) were found significant between every group (p<0.05).The microelements were lower than control group level(P< 0.05). By statistics, significant difference was revealed(P>0.05) between the levels of diabetes and control group.Statistical analysis indicated that the differences were found in WC,WHR and AMC(P< 0.05). And the differences were found in groups of D and A,B,E(P <0.05).There were significant differences in WHR among the groups of B,D,E and A (P< 0.05),and the WHR significantly differed between the groups of B,C and A (P<0.05). The differences of AMC was found among the groups of A B,E and D(P<0.05).But there were no differences in hip circumference(P>0.05).1 The level of FPG is a factor of energy metabolism.2 The level of FPG is associated with metabolism of substrate utilization on main nutrients. The persistent higher glucose is the main reason of self-preservation.3 The indirect calorimetry has distinct value in assessing metabolism in diabetes. It can be used in the treatment of diabetes.3 The patients of type 2 diabetes have malnutrition.4 Most of type 2 diabetes are overweight, and the waist circumference has relationship with control of blood glucose.
