Objective To investigate the effect of different patterns of intermittent hypoxia of sleep apnea (SA) model on the levels of interleukin-6 (IL-6) and interleukin-8 (IL-8) in vivo and vitro.Method The experiment was divided to two parts.The first part:Human umbilical vein endothelial cells of the line ECV304 were used to set up the cells model. The experiment cell strains contained one control group and eighteen experimental groups.1 constance normoxia group (control group), 3 different intermittent hypoxia(IH) degree groups (10%O2 group,1.5%O2 group, 0.5%O2 group with 12time/h),5 different intermittent hypoxia(IH) frequency groups(40time/h,20time/h,12time/h,9.23time/h and 6.32time/h) to each IH degree group. They were putted into a program-controlled gas delivery system with different level and frequency of IH respectively. Then Enzyme Linked Immuno Sorbent Assay (ELISA) was used to examine the levels of interleukin-6 (IL-6) and interleukin-8 (IL-8)The second part:All cases were collected by monitoring of polysomnography (PSG). The groups contained one control group (group A) and four experimental groups. Four experimental groups contained mild OSAS (group B), moderate OSAS (group C), severe OSAS (group D) and severe OSAS with hypertension (group E). In the morning of finishing PSG, blood specimens were collected in all groups and then ELIS A was used to examine the levels of IL-6 and IL-8 in serum.Results The first part:â‘ Higher level of IL-6 and IL-8 were found in 3 different IH degree groups than in control group(P<0.05).The level of IL-6 and IL-8 was gradually increased with the decreasing of IH degree (P<0.05).The different of IL-6 and IL-8 levels between each two different groups in IH degree was significance (P<0.05).â‘¡In 10%O2 IH degree,except the different between frequency group 3 and group 4 was not significant (P>0.05), the levels of IL-6 were higher in other IH frequency groups than in control group (P<0.05). There was no statistically significance on the level of IL-8 in frequency group 1 and group 2, group 1 and group 5, group 2 and group 5, group 3 and group 4, as well as group 4 and group 5 (P> 0.05). There was a generally trend as the IH frequency decreased, the level of IL-6 and IL-8 was tenderly increased, and highest in 12/hour group (P<0.05), gently decreased again. There was statistical significance between other each two different frequency IH groups(P<0.05).â‘¢In 1.5%O2 IH degree, the level of IL-6 was statistical significance between each two different frequency IH groups(P<0.05). There was no statistically significance on the level of IL-8 in frequency group 2 and group 5, group 3 and group 4, as well as group 4 and group 5 (P>0.05).There was a generally trend as the IH frequency decreased, the level of IL-6 and IL-8 was tenderly increased, and highest in 12/hour group (P<0.05), gently decreased again.â‘£There is statistical significance between each two different frequency IH groups (P <0.05), both on the level of IL-6 and IL-8. There was a trend as the IH frequency decreased, the level of IL-6 and IL-8 was tenderly increased in 20/hour group (p< 0.05), highest in 12/hour group(P<0.05), decreased in 9.23/hour group(P<0.05), and decreased further in 6.32/hour group (P<0.05) in 0.5%O2 IH degree.The second part:â‘ Contrasted with control group, the level of IL-6 and IL-8 in different OSAS groups and severe OSAS with hypertension group was statistically significant (P<0.05).â‘¡There was statistical significance between each two different groups on the level of IL-6 and IL-8(P<0.05). As the degree of OSAS increased, the level of IL-6 and IL-8 increased. Higher level of IL-6 and IL-8 was found in severe OSAS with hypertension group than in severe OSAS group(P<0.05).Conclusion Intermittent Hypoxia of SA caused inflammatory reaction significantly in vivo and vitro. And the level of inflammatory reaction depended on the degree of hypoxia. There was a trend as the level of IH frequency decreased, the level of inflammatory reaction was increased at first gradually and then decreased in same way. The level of inflammatory reaction increased as the degree of OSAS increased. And the level of inflammatory reaction was higher in severe OSAS with hypertension group than in severe OSAS group.
