| Abstract:Objective:To investigate the effect of intraoperative interorganizational injection chemotherapy targed on ki-67 and apoptosis in metastatic lymph nodes of advanced gastric carcinoma and to talk over its effectiveness as the strengthening treatment in the surgery of advance gastric cancer. Methods:During the surgery we resected 1 node from the No.3, No.6 or No.7 lymph nodes of the stomach to confirm someone that had lymph node metastasis in a rapidly frozen section. Those who had lymph node metastasis used as the research objects.28 patients with the condition who were randomly divided into two groups (treatment group and control group), for the treatment group a mixture of methylene blue and mitomycin(MMC) was injected in the tumor tissue or its surrounding (methylene blue 2ml MMC 8mg0.9% NS20ml) before the operation, the direct surgery for control group.After the operation metastatic lymph nodes obtained were fixed with 4% paraformaldehyde solution,paraffin-embedded and stained by HE, then each group 20 metastatic lymph nodes be selected for the study. The specimens (including the radical former specimens) were observed the effectiveness and detected ki-67 expression, cell apoptosis.Results:Before the treatment, the effectiveness of tumor cell apoptosis and necrosis was no significant difference in metastatic lymph nodes of the two groups(P>0.05). Before the treatment, ki-67 expression,apoptotic index in the metastatic lymph nodes of the two groups was no significant difference (P>0.05). After the treatment, the effectiveness (rate) of tumor cell apoptosis and necrosis for treat group is higher than those in the control group, the two groups was statistically significant difference(p< 0.05). After the treatment,the Ki-67 positive expression rate in metastatic lymph nodes of the treatment group is lower and tumor cell apoptotic index is higher than those before the treatment, statistically significant difference between before and after(p<0.05).After the treatment, Ki-67 expression in metastatic lymph nodes of the treatment is lower and tumor cell apoptosis index is higher than those in the control group, the two groups was statistically significant difference(p<0.05). Conclusion:Interorganizational injection chemotherapy targeted can inhibit tumor cell proliferation, promot tumor cell apoptosis and has a direct therapy for metastatic lymph nodes of advance gastric cancer. It plays an important role for interorganizational injection chemotherapy targeted in intensive care of advance gastric cancer during the surgery.It may have a role to prevent the iatrogenic spread and recurrence,metastasis after the surgery. |
