Objectives: To analyse the distribution of retinol binding protein 4(RBP4)gene polymorphism in HengYang.To approach the relationship between RBP4 rs375 8539,G>A gene polymorphism and Type2 diabetes mellitus as well as IGR. To approach RBP4 rs3758539,G>A gene polymorphism and insulin resistance.Methods: Two-hundred and thirty six Type2 diabetic patients,one-hundred and twenty IGR patients(83 IGT patients and 37 IFG patients) and Two-hundred and sixteen normal controls were selected randomly. T2DM and IGR were diagnosed referred to WHO standard of 1999. 3ml blood was taken from the elbow vein of every subject. Human genomic DNA was extracted by chloroform method. Genotyping for the RBP4 gene rs3758539 polymorphisms was identified by PCR-RFLP. Clinical characteristics such as HbA1c, BMI, TC, TG, BP, HDL-c, LDL-c, FBG, HOMA-IR were measured and calculated meanwhile. According to BMI, Type2 diabetic patients and normal controls were divided into normal controls with normal weight, normal controls with hypergravitation and obesity, Type2 diabetes mellitus with normal weight, Type2 diabetes mellitus with hypergravitation and obesity eventually.All the data was tested by normality test. Normally distributed data was presented as mean±SD. Non-normally distributed data was presented as median (quartile range). Group comparison between measurement data was carried out by group t-test. Group comparison between numeration data was carried out by Chi-square analysis. The relationship between RBP4 gene polymorphism and T2DM, IGR, was analysed by logistic regression analysis. The relationship between RBP4 gene polymorphism and HOMA-IR was analysed by multiple linear regression.Results: (1) FBG, HbAlc, BP, LDL-c, BMI,HONA-IR levels of Type2 diabetic patients and IGR patients were higher than normal controls (P<0.05), there were significant difference between patients with type 2 diabetes and patients with IGR; Patients with Type2 diabetes had much higher levels of TG and TC than those of normal controls significantly (P<0.05). (2)The RBP4 rs3758539,G>A gene allelic frequencies of normal controls in HengYang was 14.7% in healthy subjects, while the allelic frequencies for patients with type2 diabetes and IGR were 14.2% and 16.3% respectively; There was no statistics significance among these three groups on frequencies of the allelic gene and genetyping (P>0.05); The RBP4 rs3758539, G>A gene allelic frequencies of Type2 diabetes mellitus with normal weight patients in HengYang was 10.7% in han people, while the allelic frequencies of patients with type2 diabetes as well as hypergravitation and obesity was 18.9%. There was statistics significance between these two groups on the allelic gene (P=0.01). (3)The level of HOMA-IR of patients with type2 diabetes was higher in genetype GA+AA than in genetype GG(P<0.05).There was relationship between IR and genetype GA+AA,BMI,FBG,TC.Conclusion: (1)There is no relationship between RBP4 rs3758539,G>A and Type2 diabetic patients and IGR patients. (2)The polymorphism of RBP4 rs37585 39,G>A may be an association facter between Type2 diabetes mellitus and obesity. (3) There is relationship between insulin resistance and genetype GA+AA. It plays a importent role in affectability to type2 diabetes mellitus and increases the level of hypercholesterol, BMI as well as FBG.
