Objective:To investigate the serum level of TNF-α,IL-2 and AKBR during internal biliary drainage in acute severe obstructive jaundice rat, analyze the correlation about them, compare the role of AKBR and Child-pugh in preoperative biliary drainage for the recovery of immune functionMethods:Eighty male adult Sprague-Dawley rats were randomly assigned to three groups:LCD(ligating common bile duct) group(n=10),ID (internal biliary drainage) group,(n=60),normal group (n=10), Both ID and LCD were estabilished by ligating common bile duct (LCD), while ID was created internal biliary drainage after the first operation 7 days. On the 7 th day after the first operation, Ten LCD rats were succumbed and the specimens of blood and liver were collected;On the 7th,14th,21th,28th,35th,42th day after the second operation, Ten ID rats were succumbed and the specimens of blood and liver were collected respectively; normal group did not do any operation. The concentrations of serum IL-2 and TNF-a were measured with enzyme linked immunosorbent assay(ELISA), AKBR was measured with Enzymatic methodResults:The venous serum level of TNF-a and IL-2 were 1320.58±82.15(ng/L) and 45.12±12.65(ng/L) respectively,the AKBR was 0.432±0.042 in the LCD group rats after 7d, Those.were different significantly compared with that in the normal group (P<0.01); In ID group, after the second operation 7 days, The serum level of ALT and bilirubin were 7.25±1.16(IU/L) and 51.03±7.52(u mol/L) respectively, which decreased significantly compared with those in the LCD group (P<0.01), but no variability7 compared with those in the normal group (P>0.05); while the surem level of TNF-α,IL-2 and AKBR in ID group, whose Child-Pugh being B grade, did not become normal level, which was significant difference comparing with those in the normal group (P<0.01). The surem level of TNF-αand IL-2 became normal level after internal biliary drainage 7 days and 28 days respectively, AKBR became normal level after internal biliary drainage 35 days, so none variability compared with those in the noral group (P>0.05). The surem level of TNF-αand ALT were well correlated (r=0.762, P<0.01);the surem level of AKBR and IL-2 were well correlated (r=0.992, P<0.01) during internal biliary drainage in acute severe obstructive jaundice.Conclusions:Preoperative internal biliary drainage for the recovery of immune function plays an important role in acute severe obstructive jaundice.AKBR may be a more sensitive indicator of immune function in acute severe obstructive jaundice by internal biliary drainage than Child-Pugh... |
