| Objective:To establish the evaluation indexes system of rural community health service (CHS) capability which can be applied for the practice and is suitable for the development status in depressed area. Draw up the weight value of each index. To build the basis for managing the rural CHS in depressed area in a scientific, standard and normal way. To evaluate the rural CHS capability objectively and promote the gradual equalization of basic public health service.Methods:Combining the current situation of rural CHS and referring to literature at home and abroad, the focus group discussion method was used to draw up the draft of the evaluation indexes system of rural community health service capability with the general framework from four aspects of the infrastructure, the basic function, the management and training and the demander. The improved Delphi method was used to select the indexes, and to analyze the consultation results and information feedback. Analytic Hierarchy Process (AHP) and Rank Sum Ratio (RSR) were comprehensively adopted to calculate the weight value of indexes, and CR was used to judge and analyze the consistency. The pilot townships and villages carrying out rural community health service were chosen to do the evaluation. Crobach's alpha coefficient and factor analysis was applied to test the reliability and validity of the evaluation indexes system separately.Results:1. Experts participating in the formulation of evaluation indexes system: eleven experts engaging in rural health and CHS were selected in Jiangxi province; the available response rate of two round inquiry was both hundred percent; the authority score of experts was 0.784 and 0.803 seperately; the range of variation coefficient in second round was from 0.000~0.478 and the coordination coefficient of center and station was 0.339and 0.354 separately.2. Composition of the evaluation indexes system: the evaluation indexes system of rural CHS capability in depressed area was developed in four aspects of the infrastructure, the basic function, the management and training and the demander, and from fifteen points of views including personnel allocation, business room, section installation, equipment allocation, prevention, health care, basic medicine, health education, rehabilitation, family planning guidance and new rural cooperative medical scheme, health file management, administrative situation, training situation and degree of satisfaction. (2) CHS station: the weighting of the classâ… indexes was 0.3091,0.4074,0.1150 and 0.1685separately; the weighting of the classâ…¡indexes was 0.0983,0.0682,0.0587,0.0839,0.0619,0.0542,0.0615,0.0354,0.0482,0.0395,0.0531,0.0536,0.0604,0.0546 and 0.1685separately.4. Validity of the evaluation indexes system: the factor loadings of fifteen classâ…¡indexes of CHS centers and stations which were greater than 0.5 were satisfied, so the cantract validity of the evaluation indexes system was good. The content validity of the evaluation indexes system was also good.5. Reliability of the evaluation indexes system: (1) CHS center: the Crobach's alpha coefficient was 0.877. (2) CHS station: the Crobach's alpha coefficient was 0.873. So the reliability of the evaluation indexes system was good.Conclusions:1. The authority score of experts participating in the improved Delphi method for selecting evaluation indexes system was relative high and the coordination extent of experts'advice was relative good. Therefore, the prediction precision of study was relative high and the outcome was reliable.2. The rural CHS institutions should be equipped with necessary service condition and improve the"Six in one"based service function to carry out preliminary treatment of commonly and frequently encountered disease and to meet the health service demand of the rural residents. Therefore, the evaluation indexes system has relatively reasonable structure and complete content. 3. On the basis of experts'judgment, the combination empower method consist of the subjective empower (AHP) and the objective empower (RSR) were comprehensively adopted to determine weight coefficient to ensure the accuracy of calculated results.4. The reliability and validity of evaluation indexes system were good, so it can evaluate the rural CHS capability objectively.
