| Objective:To investigate the status and development of mental health about 6-16 years old children and adolescents,and to compare the the status and development of mental health of the singleton and the twins.Method :A total of 433 twins and singleton aged 6 to 16 years were recruited with support from educational committees and schools. After the guardians of these twins had signed an informed consent form,the Chinese version Development and Well-Bing Assessment(DAWBA) and the strength and difficulties questikonnaire were completed by the parents of these children and adolescent to investigate their mental health status.Results:A total of 443 children and adolescent were recruited, including 116 pairs of twins (232 persons), 211 singleton. 238 girls, 205 boys.the year of a total of 265 children and adolescent is more than 11 years, the year of a total of 178 children and adolescent is less than 11 years. the 443 SDQ parents assessment version were collected,a total of 268 self-assessment version,the parents assessment of SDQ that the computer export the outcome were 433(97.7%), self-assessment of SDQ were 262(97.8%), parents assessment of SDQ of the twins were 214(92.2%), self-assessment of SDQ were 98.the valid questionnaires of parents assessment of SDQ of the singleton were 204 (96.7%), A total of 164 vaild questionnaires of self-assessment.the results from SDQ computer diagnostic screening showed that there were some children and adolescent have any psychiatric disorder were up to 33.1% of 433 children and adolescents,the percent of emotional disorders were 8.8%, the percent of conduct disorder were 21.4%, there is ADHD / hyperkinesis were 14.5%. the SDQ screening of the Twin Group showed that the percent of emotional disorders were 6.6%, conduct disorder were 24.2%, ADHD / hyperkinesis were15.3% and other symptoms of mental disorders were 35.1%;the singleton group, there is percent of 11.1 were emotion disorder, and the conduct disorder were 18.4%, ADHD / hyperkinesis were 13.5%,the percent of other mental disorders were 30.9%.the results of the parents assessment and self assessment of the SDQ of Wilcoxon signed-rank test on two groups showed that there were differences between the two groups on peer problems and pro-social (P <0.05 ), the anothers factors were not significantly different. Afte comprehensive two versions of the SDQ derived from the computer ,the result showed that there wer no difference between the two groups (P> 0.05). Two versions of the computer on DAWBA comprehensive analysis of the data obtained computer diagnosis, the results for the 443 people who also meet the diagnostic criterion of DSM-IV and ICD-10,the diagnosises includes that there were 1 person had autism(0.5%), 2 person were separation anxiety disorder (0.5%), generalized anxiety diagnosis were 1 (0.2%),on parent version of DAWBA ,the percent of singleton group more than the twins group. including generalized anxiety, obsessive compulsive, depressive symptoms,deliberate self hurt, attention deficit hyperactivity disorder and oppositional defiant/conduct disorder. The percent of another disorder in the twins group were more than in the singleton groups.the self-assessment of DAWBA computer diagnosis, the percent of symptoms of twins group was higher than the singleton group, including the generalized anxiety symptoms, depressive symptoms and conduct disorder symptoms and the impact of the symptoms, the anothers in the sub-clinical symptoms and its effects on mental health issues on the singleton group were lower than the twins. the parents assessment of DAWBA computer diagnosis result of the Wilcoxon signed-rank test on the two groups showed that there was significant different between the group of twins and the groups of the singleton (P <0.05),including symptoms of the special terror effects symptoms and impact of generalized anxiety, social phobia symptoms and impact, depression, ADHD, oppositional defiant/conduct disorder symptoms,tic symptoms and other concerned.on the Self-assessment version of DAWBA computer diagnosis,there was significant different between the singleton and twins(P <0.05),including the symptoms and the impact of the special terror, social phobia symptoms and its impact, post-traumatic stress disorder symptoms, obsessive compulsive, tic symptoms, eating disorder symptoms and other concerned symptoms. The parent version of the SDQ and self-assessment with the Kappa test, the total of difficult between in the parent version and self-assessment version with the kappa was 0.223 (P <0.05), mood disorders between in the parents version and self-assessment version with the kappa was 0.242 (P <0.05), behavioral problems between in self-assessment version and the parents version of kappa was 0.166 (P <0.05), hyperactivity / attention deficit disorder between in the parent version and self-assessment version with the kappa was 0.129 (P <0.05), peer problems between in the parent version and self-assessment version with the kappa was0.175 (P <0.05), the pro-social between in the parent version and self-assessment version of kappa was 0.066 (P> 0.05), symptoms affecting the parent version and the kappa between self-assessment version coefficient was 0.242 (P <0.05). Two versions of the consistency hypothesis testing, the results indicated that the total difficulties, emotional disorders, behavioral problems, hyperactivity / attention deficit disorder, peer problems and pro-social effects of parental symptoms and self-assessment version version has between consistency (P <0.05). DAWBA parent version and self-assessment version of consistency tests have shown that separation anxiety symptoms, kappa was 0.227 (P <0.05), separation anxiety affect kappa value was 0.004 (P> 0.05). Kappa value of specific terrorist symptoms 0.183 (P <0.05); special effects terror symptoms kappa value of 0.114 (P <0.05). Symptoms of Generalized Anxiety kappa value of 0.339 (P <0.05); generalized anxiety symptoms affect the kappa value was 0.665 (P <0.05). Social phobia symptoms kappa value of 0.253 (P <0.05); social phobia symptoms in kappa value was 0.231 (P <0.05). Panic symptoms kappa value of -0.007 (P> 0.05). Post-traumatic stress symptoms kappa value 0.278 (P <0.05). Kappa value was 0.239 on depressive symptoms, symptoms of the parents in this version there is consistency with the self-assessment version (P <0.05). Other symptoms of concern kappa = 0.000, on the parent version of this symptom and self-assessment version of the same does not exist (P> 0.05). Two versions of the consistency hypothesis testing,the results showed that there was consistency between the parent assessment and the self-assessment of DAWBA(P<0.05),including in separation anxiety symptoms, symptoms of terror and its special effects and impact of generalized anxiety, social phobia symptoms and its impact, post-traumatic stress symptoms of depression and other symptoms.Conclusion:433 children and adolescents by the SDQ and DAWBA screening, there were the higher detection rate in mental health disorders or psychological symptoms of subclinical symptoms on some children and adolescents.there were consistency between parent assessment and self-assessment version, but the consistency is not high, parents evaluate their children and their children on their own evaluation of certain degree. there was no significant difference on SDQ computer diagnosis of the symptoms between the singleton and the twins, there were significant differences about some symptoms of DAWBA computer diagnosis between singleton and twins, some of the detection rate of sub-clinical symptoms of twin group compared with singleton group was higher. The Number of children can not be considered the decisive role on mental health and development of children. |
