| Objective: Discuss the etiology, clinical manifestation, diagnosis, treatment and prognosis of senile patients with acute obstructed suppurate cholangitis (AOSC), further to improve the diagnosis and treatment of senile patients with AOSC.Methods: Retrospective analysis of the clinic characteristic and the ways time and complications of surgery of 56 senile patients with AOSC (older than 60 years old) in our hospital from march 1999 to march 2009.Results: Compared to the others group, the old group has its own manifestations: symptoms appear later; milder abdominal pain, but more severe signs; higher morbidity of septic shock and multiple organ dysfunction syndromes (MODS) will rapidly aggravate; more basically diseases are companied with. There are 47 surgery patients, 42 of them were cured, the other five died, and the mortality is 10.64%; for the nine non-surgery patients, three of them were cured, six died, and the mortality is 66.67%.Conclusion: The senile patient has its own manifestations: milder symptoms, but more severe signs; more complication, more misdiagnose .Septic shock and MODS are the main causes of death of senile patients with AOSC. Positive preparation for surgery ,operation as soon as possible, releasing the obstruction of bile duct, drainage, controling the inflammation and anti-shock are the key ways to deal with the senile patients with AOSC. |
