| Objective:To explore the clinical effect of orthotopic sigmoid colon neobladder after radical cystectomy and to provide urodynamic parameters. Methods:32 patients with invasive bladder carcinoma undergoing laparoscopic radical cystectomy with orthotopic sigmoid colon neobladder were examined urodynamically.Evaluations included maximum urinary flow rate,postvoid residual,filling and voiding cystometry, bladder volume.Clininal. Record patients 3 months, 6 months, 12 months of day and night continence and the rate of urinary incontinence. Results:Three month after operation the male'scystometric capacity,the pressure of storage and voiding,the maximum urinary flowrate was 346.2ml(248~440ml),24.9cmH2O(18~35cmH2O),63.3cmH2O(43~80cmH2O),16.1ml/s(11~19ml/s). And sixth month the cystometric capacity,the pressure of storage and voiding,the maximum urinary flowrate was 420.6ml (310~490ml),20.0cmH2O(16~26cmH2O),69.9cmH2O(50~89cmH2O),19.5ml/s(14~24ml/s).The cystometric capacity,the pressure of storage and voiding,the maximum urinary flow rate between the 2 groups were significant(P<0.05). Three month after operation the female's cystometric capacity,the pressure of storage and voiding,the maximum urinary flow rate was 270.1ml (210~346 ml), 31cmH2O(29~33cmH2O), 60.5cmH2O (45~69cmH2O), 10.8ml/s(9~13ml/s). Sixth month after operation the cystometric capacity,the pressure of storage and voiding,the maximum urinary flow rate was 312.5ml (270~395ml), 24.4cmH2O(20~31cmH2O), 62.7cmH2O (45~86cmH2O), 16.2ml/s(13~19ml/s).The cystometric capacity,the pressure of storage and the maximum urinary flow rate between the 2 groups were significant (P<0.05) and the pressure of voiding (P>0.05). Conclusions:Radical cystectomy with orthotopic sigmoid colonic neobladder can be a feasible and effective method for the treatment of bladder cancer. Further, New bladder was satisfactory with continence and micturition.Investigations are needed to study the longterm outcomes of orthotop sigmoid colon neobladder. |
