As adolescents are susceptible groups for depression, more and more researchers emphasize on understanding of their depression occurrence. Domestic research instruments on adolescent depression are mainly derived from the west, lacking localized depression measure scales. Moreover, researches on the characteristics of adolescent depression are unsystematic. On the basis of document review and expert suggestions, this study aims to develop adolescent depression self-report questionnaire. Having been applied to 901 adolescent students, questionnaire's reliability, validity and critical grade were assured by several test methods. Results are as follows:(1) Adolescent student depression self-rating questionnaire consists of three dimensions: negative mood, sense of uselessness, and physical symptoms. It includes 30 items, adopts 4 point score for self rating, and the time span it involves is a week.(2) The reliability and validity tests indicate that this questionnaire has good internal concordance coefficient, split-half reliability, and retest reliability, and content validity, construct validity, and index validity, thus it is applicable for adolescent student depression survey.(3) With SDS as standard, the critical grade of adolescent student depression scale is 58, and the adolescents student depression occurrence is 31.19%(4) Adolescents student depression occurrence differ significantly among grades. Gender differences within a single grade are inconsistent.
