| Knee osteoarthritis (KOA) belongs to osteoarthritis. KOA is in a kind of chronic progressing arthrosis disease and occurs mostly in older people (especially in older wo men, which is also called retrogressive arthritis, deformed arthritis,atrophic arthritis,knee hyperplasia arthritis, retrogressive osteoarthrosis. KOA is a common clinical orthopedics, which seriously affecting people’s daily life and work.Clinical work found that many KOA patients associated with knee varus deformity; Its main clinical menifetation is progressing chronic arthragia, rigidity, intumescince accompanied with movement restriction, which can lead to dysfunction of joint, even deformity and disability in svere cases.,which Brings great difficulties to the treatment.Body gravity mostly focused on the medial knee joint due to the lawer limb abnormal axis,which causes the medial joint space narrow and damage medial cartilage.If the situation continued for a long time it creating a vicious circle, which Leading to conservative treatment is poor, and easy to relapse.However,TKA exists many problems.such as large operation wound and risks, the high cost has become a huge burden on the patient, so that many patients are unwilling to accept.The high tibial osteotomy (HTO)correcting the line of abnormal lower limb force, so that the load line of the joint is transferred from the medial knee joint to the center and the lateral knee joint, localized point load into weight-bearing surface, thereby reducing the load of the medial joint and reduce or eliminate the cartilage wear,f so as to relive pain and improve function and delay joint degeneration. During clinical practice,we found that HTO is noy only correcting the line of abnormal limb force,promoting the formation of the new joint surface but also relive or eliminate pain and improve or restore function. in addition,HTO also has other advantages like less trauma,low-risk, simple surgical procedures,lower compilications and high safety.ObjectiveTo study and analyze the clinical effect of High level tibial osteotomy therapy for the treatment of varus knee with osteoarthritis.MethodsIn strict accordance with the inclusion criteria and exclusion criteria in patients with Knee osteoarthritis with varum collected in October 2013-2016 in January during the Third Affiliated Hospital of Beijing University of Chinese Medicine hospital treatment,34 cases(51 knees) with varus knee with osteoarthritis were treated with the HTO. The therapeutic effects were assessed by comparing the knee function scores of before and after lyear surgery according to HSS knee joint criteria scoring system,Comparing the changese of FTA. Finally the use of statistical software sass6.12 related statistical analysis.ResultsPreoperation:According to HSS criteria,34casas(51knees)got the scores of 36 to 69 points, an average 53.18 ± 9.10 points; FTA were 178 °to 192°, an average 183 ° ± 5 °.1 year after surgery:According to HSS criteria,34casas(51 knees) got the scores of 66 to 93 points, an average 81.41 ± 6.76 points, with excellent in 31 knees, good in 16 knees, fair in 4 knees and poor 0 knees,with the excellent and good rate of 92.15%. FTA were 171 °to 176 °, an average 173 ° ± 3 °. Before and 1 year after surgery, comparing changes of HSS scores and FTA,there were significant differences(P<0.01)Conclusion1.The high tibial osteotomy can apparently improve the knee function of Knee osteoarthritis with genu varus,and follow-up short-term positive effect.2.The high tibial osteotomy can apparently correct the weight loading axis of Knee osteoarthritis with genu varus. |
