The main symptoms are cough, with or without fever, sputum and chest pain. It has high incidence and is serious harm to human health. In recent years, due to irrational use of antibiotics, the result is in poor treatment of CAP. Bacteria, especially Gram-positive cocci are the main CAP pathogen infection, but in recent years, the proportion of CAP caused by atypical pathogens and viruses are on the rise. With the outbreak of SARS, H1N1 and other diseases, medical treatment for respiratory tract infections attracted more and more attention. Traditional Chinese medicine have significant advantages in prevention and treatment of bacterial infections of respiratory system disease. It has an advantage over Western medicine, in terms of prevention and treatment of viral and atypical pathogens causing respiratory tract infections. CAP can diagnose and treat in the reference of air temperature Hyperactivity disease, and the basic pathogenesis is excessive noxious heat, phlegm in the lungs. Treatment should focus on detoxification, phlegm. The treatment should according to detoxification, phlegm Xuanfei medication. Qingfei detoxification agent is the hospital preparation of Beijing University of Chinese Medicine, Dongzhimen Hospital. It has a good effect in the treatment of CAP caused by a variety of pathogens. But now we lack its research, on clinical aspects and mechanisms.Objective:(1) summarizes the characteristics of CAP t in recent years, in Dongzhimen Hospital, in order to seek detoxification treatment Qingfei Mixture CAP research foundation; (2)To study the clinical efficacy of detoxification Qingfei Mixture treatment of non-severe CAP by using randomized controlled method;(3) To study the Mechanism of detoxification Qingfei Mixture treatment of CAP, to provided the theoretical basis of Chinese medicine prevention of respiratory infectious diseases.Methods:(1) Finding out the statistics of patients with pulmonary infection with a total of 1389 cases, through the information Center of Beijing University of Chinese Medicine, Dongzhimen Hospital,time interval between 2012.5-2015.4.453 cases met the inclusion criteria, and collect their basic statistical information, syndrome, etiology, diagnosis and treatment of traditional Chinese medicine, etc.(2)Formulating the further research programs, according to the results of medical research. Researching thepatients with lung infection who met theinclusion criteria,between 2015.5-2015.12,in Dongzhimen Hospital. Dividing into detoxification Qingfei Mixture treatment group and conventional treatment group, and comparing before and after treatment in each group on symptom scores, signs, blood and other physical and chemical indicators; (3) detecting the serum levels of TNF-a, IL-6 and IFN-γin CAP patients, explore the immune inflammatory injury adjustment mechanism ofthe CAP patients treated by Detoxification Qingfei Mixture.Results:(1)Retrospective The most common type is phlegm obstruct the lung.Maxingshigan soup is the most frequently used Chinese Herbs. The main clinical symptoms show significant improvement in 4-7 days. These characteristics are the basis of clinical studies.(2) Clinical efficacy:Syndrome Evaluation,efficiency was higher in the treatment group, P<0.05, the difference was statistically significant, indicating detoxification Qingfei Mixture for treatment of non-severe CAP effective, and in some cases have obvious advantages; the efficacy of a single symptom, fever time in the treatment group is better than the control group, P<0.05, the difference was statistically significant; on remission time of cough and sputum,the treatment group is better than the control group, P<0.05, the difference was statistically significant; cough sputum symptoms improve efficiency in the treatment group is better than the control group, P<0.05, the difference was statistically significant. Description detoxification Qingfei Mixture has obvious advantages on mitigation fever and cough.(3) Experimental index:Serum CRP levels within the group, CRP levels were decreased, P <0.05, having significant difference in the treatment group and the control group; between the two groups, the treatment group decreased levels is better than the control group, P<0.05, the difference was statistically significant. Within the group of serum IFN-y levels, in the treatment group and the control group had no significant difference before and after treatment levels, P> 0.05, the difference was not statistically significant; the groups were compared, no significant difference in the level ofthe two groups, P> 0.05, the difference was not statistically significant. Within the group of serum IL-6 levels in the treatment group and the control group after treatment, levels were decreased, P<0.05, the differences were statistically significant; between the two groups, there are significant differences in the degree of decline in the treatment group and the control group, P< 0.05, the difference was statistically significant. Comparison of serum TNF-a levels in the treatment group and the control group,after treatment the levels were decreased, P<0.05, the differences were statistically significant; the groups were compared, there are no significant difference in the degree of decline in the treatment group and the control group, P> 0.05 the difference was not statistically significant.Conclusions:(1) CAP is mainly phlegm retention in the lung, Maxingshigan soup is effective in clinical treatment;(2) detoxification Qingfei Mixture can effectively control non-severe CAP; (3) Detoxification Qingfei Mixture has effective treatment of non-severe CAP, by inhibiting inflammatory cytokines.
