Evaluation Of The Prognosis Of Diffuse Large B-cell Lymphoma Using Mid-term 18 F-FDG PET / CT And Deauville Scoring Criteria

Objective:The aim of this paper is to use a newly and internationally recommended Deauville 5-point score criteria in PET reporting. The results of interim scan is divided into two groups:negative group(1-3 points) and positive group(4-5 points). The interim prognostic evaluation applicability of this criteria in DLBCL will be tested and inspected in this research.Materials and Methods:79 in-patients from Jan,2008-Dec,2015 in Nanjing Drum Tower Hospital are selected. All the patients are retrospectively analyzed.47 are male and 32 are female, aging from 13~86 years old(46 in average).5-38 months’ follow-up records are reviewed. The PFS of both Deauville score positive group and negative group are compared to each other. The results of PET/CT and single CT are also compared respectively.Results:After 21 months’median follow-up in totally 79 patients,61 patients(77.2%) are negative with remission. The rest 18 patients(22.8%) reveals positive, among which 12 patients are complete remission(CR) after treatment change till end to the follow-up and 6 patients reveal treatment failure(2 were died of complications). In the 61 negative patients,54 are complete remission(CR) after first line therapy;5 of them relapse in 2 years and 2 of them turn out failure. The positive group owns a 33.3% failure percentage while the negative owns 3.3%, which are statistically significant. 79 DLBCL patients have a 2-year PFS of 79.7% and a 2-year OS of 84.8%. The 2-year PFS of the negative groups and positive group are 83.6% and 66.7%. The single factor analysis shows F-FDG PET/CT is the risk factor of 2-year PFS.Conclusion:Deauville score criteria reveals a highly predictive value in the interim treatment outcome assessment and prognostic prediction. We appeal that by adding Deauville score criteria into normal clinical PET report is essential and it will provides a strong assessment approval.
