Study On The Susceptibility Of Vitamin D And VDR Gene FokI, BsmI, TaqI, ApaI Polymorphism To Type 2 Diabetes Mellitus And Diabetic Nephropathy In Yunnan Han Nationality

[Objective] Our study is aim to research the concentration of 25 hydroxy vitamin D in Yunnan Han population, then to probe the relationship between vitamin D and diabetic nephropathy.[Methods]we recruited 273objects of han population of Yunan, which included 90 type 2 diabetes patients (without diabetic Nephropathy),88type 2 diabetes accompanied by diabetic Nephropathy,and 95 healthy controls. We comparethe concentration of 25 hydroxy vitamin Din those 3 groups,and study therelationship between the concentration of 25 hydroxy vitaminD in patients’ plasma and diabetic Nephropathy. [Results] (1)The plasma concentrations of 25 (OH) D of healthycontrols is significantlyhigher thantype2 diabetes group(P<0.01); (2) The plasma concentrations of 25 (OH) D of healthy controls is significantly higher thandiabetic Nephropathy group(P<0.01)(3)Vitamin deficiency (12-20ng/ml)has a weak relationship withNephropathy group;(4)There is significant difference between type2 diabetes group and Nephropathy group in eGFR;(5)When grouped according to eGFR in Nephropathy group, there is no difference in vitamin D on each group;(6)The plasmaconcentrations of 25 (OH) D has positive relation with HDL, while has negative relation with HbA1c%、BMI、LDL、TG;(7)LDL (P<0.01)、 course of disease (P<0.01)、age (P<0.01)、SBP (P<0.01) may be the susceptible factors for diabetic Nephropathyby Logistic regression analysis. [Conclusion]There is a downward trend in concentration of 25 hydroxy vitamin D in type2 diabetes and Nephropathy group when control with healthy objects.[Objective] Observethe distribution of vitamin D receptor gene FokI, BsmI, TaqI and ApaI in Yunnan Han population, then study the relationship between vitamin D receptor gene by and plasma concentrations of vitamin D, finally explore the susceptibility of type 2 diabetes and diabetic nephropathy. [Methods]88 patients with type 2 diabetes excluding diabeticnephropathy,89 patients with diabetic nephropathy,and 96 healthy controls were enrolled in this study. The plasma 25(OH) D level was detected by enzyme linked immunosorbent assay (ELISA). The polymorphisms (rs10735810.rs1544410. rs731236 and rs7975232) of VDR genes were genotyped by real time fluorescence polymerase chain reaction (RT-PCR) by Taq Mantechnology. [Results](1)About FokI(rs10735810):The rate of genetype FF. Ff. ff in those three groups:DM group (28.6%.54.9%.16.5%), DKD group (20.2%. 52.8%.27%), NGT group (32.3%.46.95%.20.8%),There are apparent differences between DKD group and NGT group in FF genotype (P=0.043,P<0.05),and between DKD group and DM group in Ff genotype (P=0.048,P<0.05).Genotype ff may be the risk factors of DKD by Logistic regression;(2) About BSMI:The rate of genetype BB. Bb. bb in those three groups:DMgroup (3.4%、6.8%、89.8%), DKDgroup (2.2%. 12.4%、87.4%), NGTgroup (1%、8.3%、90.7%), There are apparent differences between NGT group and DM group in bb genotype (P=0.042,P<0.05);(3)About TaqI: The rate of genetypeTT.Tt.tt in those three groups,DM group(87.9%.9.9%.2.2%), DKDgroup (84.3%.11.2%.4.5%), NGTgroup (85.4%.9.4%.5.2%) There are apparent differences between NGTgroup and DM group inT genotype (P=0.021,P<0.05);(4)AboutApaI:The rate of genetype AA、Aa、aa in those three groups:DM group (28.6%.65.9%.5.5%), DKD group (41%、50%、10%), NGTgroup (31.25%.56.25%.12.5%),There are apparent differences between DKD group and DM group inAa、aa、A genotype(P=0.014, P<0.05; P=0.031, P<0.05; P=0.037, P<0.05), and NGTgroup is different form DMgroup inAA、Aa genetype(P=0.042, P<0.05; P=0.021, P<0.05), GenotypeAamay be the risk factors of DKD by Logistic regression; (5) The four genetypes have a linkage disequilibrium, and the combination of fBAT gene may be a risk factor for diabetic nephropathy. [conclusion] Genotype ff,Aaand the genentyoe of fBATmay be the risk factors of DKD...
