The Clinical Observation For Treatment Of Steroid-induced Necrosis Of The Femoral Head With Core Decompression And Bone Graft And Traditional Chinese Medicine

Objective:To promote the restoration of cystic lesions in the femoral head with core decompression and bone graft. And observe the mechanism how chinese medicine ShengXin Fu Yuan Wan promote the restoration of femoral head by regulating endocrine and improving microcirculation through clinical research. Methods:30 patients with necrosis of the femoral head Ficatl&2 were treated by drilling decompression and bone graft through trochanter and oral Sheng Xin Fu Yuan Wan, externally apply Ding Xiang Gu Huo San.The control group were treated drilling decompression. We scored the joint function before and after the operation and fllowed up according to the Harris system. The blood lipids, blood coagulation index, blood flow changes, X-ray, BGP, urinary Ca/ Cr changes before and after treatment were compared. Results:In the treatment group,the average score is 58 before the operation and 92 after the operation, the the excellent and good rates were 86.7%and the the excellent and good rates were 92%in Ficatl and 72%in Ficat2.And in the control group, the average score is 54 before the operation and 79 after the operation, the the excellent and good rates were 63.3%t and the the excellent and good rates were 73%in Ficatl and 52%in Ficat2. There was significant difference in complications between 2 groups (P<0.05). The treatment group significantly lower cholesterol and triglyceride role, there was significant difference to before treatment (P <0.05), while in the control group before and after treatment there was no significant difference (P> 0.05).The treatment group prothrombin time in varying degrees lower, reducing the role of fibrinogen (P<0.05), while in the control group before and after treatment there was no significant difference (P> 0.05).The treatment group in reducing high blood shear viscosity, low-cut than the whole blood viscosity, the role of hematocrit blood flow changes have received varying degrees of improvement, and no significant change in the control group (P> 0.05). Three months later therer was a significant difference between the two groups of patients with X-ray comparison througe the Ridit analysis (P<0.05).There was no significant differencer between the two groups of patients before and after treatment by the statistical analysis of X-ray after one year (P> 0.05), so we can not consider that there was significant differences in X-ray signs of change between two methods in the treatment of osteonecrosis of the femoral head.The treatment group of patients BGP increased and urine Ca/Cr reduced by statistical analysis compared with the control group (P<0.05), there was a significant difference.So the method can significantly improve the BGP and has the function to promote bone formation and reduce the function of bone resorption. Conclusion:Through the clinical observe of 30 patients ShengXin Fu Yuan Wan and Ding Xiang Gu Huo San can promote the patients after surgery with symptoms and signs, laboratory indicators improve and reduce the suffering. It can promote the restoration of femoral head in the early time after suggery, but in the long time there was no significant differences in X-ray signs.
