Objective: To investigate the therapeutic effect of combined administration of Parecoxib Sodium for pre-emptive analgesia with small dose Fentanyl on postoperative analgesia in gynecologic operative laparoscopy.Method: 56 elective cases of gynecologic operative laparoscopy were selected, all subjective were characterized as follow: ASAⅠ-Ⅱ, 29-53 years age, 45-75kg in weight and 150-173㎝ in height. The patients were randomly divided into two groups: group of pre-emptive analgesia with Parecoxib Sodium(group A)and group of post-operative injection with Parecoxib Sodium (group B).There are 28 cases in each groups. In group A, 40mg Parecoxib Sodium was injected at 30minitues before operation, as well as injection of 1.0 ug/kg Fentanyl at 30minitues before the end of operation; in group B, 40mg Parecoxib Sodium and 1.0 ug/kg Fentanyl were given at 30minitues before the end of operation. VAS and sedation score were valued respectively at various times after operation. Additional doses of Fentanyl was recorded in post-operative 12hours and the following 12 hours. Complications such as nausea & vomiting, pruritus, drowsiness, respiratory depression and hematuresis etc. were also taken into account. Coagulation function and renal function were observed for all patients before injection of Parecoxib sodium, 12 hours and 24 hours after injection of Parecoxib sodium.Results: Compared the general conditions such as age, body mass ,height, operative duration, childbearing history or operative type etc, there are no statistical significance in these two groups(P>0.05). Compared to group B, post-operative bonus of Fentanyl or VAS was reduced obviously in group A, the difference is statistically significant(P<0.05). While at 13-24 hours after operation, there is no statistical significance of bonus or VAS between these two groups. Compared to the Ramsay sedation score or the incidence of complications, there is no statistical significance between two groups, either(P<0.05). No differences of coagulation function and renal function for all patients at various times before intravenous Parecoxib sodium, 12 hours and 24 hours after injection of it were observed(P>0.05).Conclusions: The combined administration of Parecoxib Sodium for pre-emptive analgesia with small dose Fentanyl on postoperative analgesia in gynecologic operative laparoscopy is clinically effective, capable of opium reduction and lower incidence of complications. However, It did not affect the coagulation and renal function.
