Objectives:To investigate the levels of iNOS/NO system and ET-1/ET in patients with chronic hepatitis patients and liver cirrhosis patients and compare the differences between them studying the effect of NO and ET in the course in which hyperdynamic circulation with hepatic cirrhosis occurs.Material and Methods:The plasm a levels of iNOS activity and ET of 20 chronic hepatitis patients,20 liver cirrhosis patients and 10 nomal controls were measured by Griess and radioimmunoassay. Part of the liver tissues was embedded and fixed for immunohistochemical analysis of the distribution of iNOS and ET-1 proteins in the livers. The slides were then analyzed with an Image Analyzing system to obtain the Gray-scale value of stained proteins. The expression of iNOSmRNA and ET-1mRNA in the liver samples was semi-quantitatively assayed with RT-PCR. The mRNA expression of GAPDH was also detected and used as an internal control. Quantity One gel imaging system was used for the semi-quantitative analysis of the mRNA expressions. The results were expressed as the ratio of the densities of the expression bands of CBS, CSE with that of GAPDH. One-way ANOVA, Independent-samples T-test and Pearson were used to determine the significance of difference among the four groups.Results:1. The plasma iNOS activity in portal venous and peripheral with chronic hepatitis and cirrhosis patients are all increased than it is in control group. (F portal=102.793, P portal< 0.01; F peripheral=25.052, P peripheral≤0.01).Furthermore, the iNOS activity in portal venous was more visible increased. The Grey of iNOS of liver with chronic hepatitis and hepatic cirrhosis are lower than the control (F=46.796, P< 0.05). The expression of protein iNOS is enhanced. The expression of iNOS mRNA of liver with chronic hepatitis and hepatic cirrhosis are all higher than control (F=26.832, P< 0.01), and the expression keeps increasing gradually with the development of the disease.2. The plasma ET-1 level in portal venous and peripheral with chronic hepatitis and cirrhosis patients are all increased than it is in control group. (F portal=103.368, P portal< 0.01; F peripheral=16.333, P peripheral≤0.01).Furthermore, the level of ET-1 in portal venous was more visible increased. The Grey of ET-1 of liver with chronic hepatitis and hepatic cirrhosis are lower than the control (F=49.872, P< 0.05). The expression of protein ET-1 is enhanced. The expression of ET-1mRNA of liver with chronic hepatitis and hepatic cirrhosis are all higher than control (F=105.384, P<0.01), and the expression keeps increasing gradually with the development of the disease. Conclusion:Vasoactive substances nitric oxdde, endothelin has an important effect which maintains the state of diastolic blood vessels on the occurrence and development with hepatic cirrhosis.
