Objectives:Tuberculosis is one of the infectious diseases which still threats People's Health heavily.Recently,the epidemic situation of tuberculosis was worsening because of the neglect of control in some areas,meanwhile drug-resistant tuberculosis,HIV infection accompanied with Tuberculosis,and so on.This study aim is to understand the genotypic characterization of Mycobacterium tuberculosis and distribution of cluster characterization of Mycobacterium tuberculosis. Methods:A.Case selection:The patients with positive MTB test were enrolled in this study from January 2007 to June 2007 in HuzhouB.Genotyping:The method of MLVA was used to genotype the clinical isolates. Briefly,15 VNTR locus were amplified by PCR which used extracted genomic DNA of MTB as templates and agarose gel eleetrophoresis.Quantity one software was used for processing image data and genetic information.C.Database:the database of tuberculosis patients and tuberculosis genotype were mamaged.D.Data analysis:DNA database of 15 VNTR locus from 180 strains was clustered by BioNumerics 4.5 software.All analyses were carried out by SPSS13.0. P<0.05 indicates statistical significance. Results and ConclusionsA.180 patients were composed of 119 male and 61 female.The age range was from 14 year - 87 years.25-44 age group accounted for 44.4%(80/180).The occupations of patients included peasants(38.3%),migrant Laborers(35.6%),commercial and Service Providers(8.3%) and others.Of 180 patients,106 were nvere untreated, 20 were retreated.B 180 strains were grouped into 7 gene groups,named as group a(2.2%),b(5.0%), c(2.2%),d(9.4%),e(9.4%),f(6.7%),g(65.0%).Group g was primary prevalent strains in this study.C.180 Mycobacterium tuberculosis strains displaying 93 genotypes,62 strains (34.4%) had the same genotypes and 118 strains(65.6%) were composed of 31 clusters.The biggest cluster had 26 strains;the other 6 clusters had 7,6,5,4,3 and 2 strains,respectively.The cluster rate was 65.6%of the 180 the Mycobacterium tuberculosis strains.D.Of 15 VNTR locus,Mtub-21 and Mtub-39 had high polymorphisms;ETR-A, MIRU-10,MIRU-40 and Mtub-30 had moderate polymmorphism;MIRU-39, MIRU-27,MIRU-26,MIRU-16,ETR-E,ETR-D,ETR-C and ETR-B had low polymorphism.E Of the 119 stains from male patients,the cluster rate was 67.2%(80/119),Of the 61 stains from female patients,the cluster rate was 62.3%(38/61 ).There were no significance between the cluster rate among male and female(χ~2=0.43,P>0.05). The cluster rate in the age group 14-44,45-64 and > 64 group was 76.9%(83/108 ), 70.3%(26/37) and 25.7%(9/26),respectively.There was not significant difference between the cluster rate among the age group 14-44 and 45-64 and 65 years or older(x2 =39.99,P<0.05;χ~2= 8.32,P<0.05). The clusters rate of migrant Laborers,commercial and Service Providers,workers and peasants was 78.1%(50/64),86.7%(13/15 ),67.0%(12/18 ) and 50.7%(35/69), respectively.There was not significant difference between the cluster rates among various occupations.The clusters rate of the age group between the stains from the untreated and retreated patients was 73.1%(117/160 )and 5.0%(1/20 ),respectively. There was not significant deference between their cluster rates(χ~2=7.06,P<0.05).Generally,180 Mycobacterium tuberculosis strains typed by MLVA.The strains of Mycobacterium tuberculosis in Huzhou present definitely gene diversity.Among 15 VNTR locus,Mtub-21,Mtub-39,ETR-A,MIRU-10,MIRU-40 and Mtub-30 locus were the optimized loci.By MLVA methods,a total of 93 different genotypes were identified by 15 locus for all 180 strains fell into 7 genetic group.g genetic group were dominant in Huzhou.The cluster rate was 65.6%of the 180 the Mycobacterium tuberculosis strains.The epidemic of situation is serious for recent transmission.
