| Objective 1.To program independent intellectual property rights of brain injury cognitive assessment and rehabilitation system software. 2. Examine the reliability and validity of the self-designed Computer-assisted Cognitive Assessment System. 3. Provide objective, simple and accurate computer-assisted cognitive assessment system and effective rehabilitation training software for clinic.Method Reference to the current existence of neuropsychological scales at home and abroad, selected the appropriate assessment items for traumatic brain injury patients, programed the operation and the scale items into computer soft. 71 and 25 patients with cognitive impairment of traumatic brain injury were assessed respectively by Mini-mental State Examination and Neurobehavioral Cognitive Status Exam,and next day assessed by self-designed Computer-assisted Cognitive Assessment System. 65 cases of normal were assessed by self-designed Computer-assisted Cognitive Assessment System,two weeks later assessed them again,4 cases of normal were lost to follow.At the same time, according to the study of cognitive rehabilitation with traumatic brain injury at home and abroad,combined with clinical symptoms of patients with traumatic brain injury, self-design ideas of cognitive rehabilitation, and programed it into computer-assisted cognitive rehabilitation system for clinical cognitive impairment in patients with traumatic brain injury.Used SPSS 11.5 statistic software to calculate test-retest reliability,split-half reliability,Cronbach's alpha coefficient, criterion-related validity, contract validity, sensitivity and specificity.Results Self-designed Computer-assisted Cognitive Assessment System, the test-retest reliability coefficients of subtests and scales ranged from 0.876 to 0.944 and 0.984 in the normal. In the normal and patients the split-half reliability were 0.855 and 0.830 , and the Cronbach's alpha coefficient was ranged from 0.773 to 0.892.The criterion-related validity compare with Mini-mental State Examination were scales 0.929,orientation 0.951, language 0.981 , calculation 0.742. The criterion-related validity compare with Neurobehavioral Cognitive Status Exam were all more than 0.8.Principal components analysis analyzed contract validity, analyzed the former eight factors, the sums of cumulative were 87.38%. According to Youden's index, the optimum cut-off point score of 41, calculated sensitivity of 1 and specificity of 0.82.Conclusions Self-designed Computer-assisted Cognitive Assessment System has excellent reliability,validity,sensitivity and specificity. It is worth to promote and use in the clinic and the self-designed Computer-assisted Cognitive Rehabilitation System has a long-term prospects for clinical use. |
