| ObjectiveOne aim of the research was to find the traditional chinese medicine(TCM) distribution of diabetic nephropathy(DN) with renal failure, and the other one was to search whether stagerelated links between TCM and Western medicine of DN, by doing a unified survey of TCM syndrome, under the guidance of TCM theory, which would guide clinical.MethodsThe thesis summarizes included two parts, the first part was the literature review and the second was clinic research.The review part included the development of morden medical and the development of traditional Chinese medical on diabetic nephropathy with renal failure.The clinical study part included the four parts below.1. Inclusion of all eligible cases were in accordance, with the unified design standard survey form to record clinical information, including:①General information:number or out-patient hospital number, name, sex, age, hospital treatment, observation time, weight, medical history, etc..②Western diagnosis included staging of diabetic nephropathy and renal failure staging.③The endogenous creatinine clearance rate represents the glomerular filtration rate.④Chinese information:clinical symptoms, signs, tongue, pulse and so on.2. In this survey, the patients were questioned accordance with uniform standards, the questionnaire content was filled out. Based on inclusion criteria and exclusion criteria,80 patients with complete data were admissed for during the period of 2009-2011 in the Dongfang Hospital and Dongzhimen Hospital. The database was locked when data had been ensured correct.3. SPSS18.0 software was applyed for data statistical analysis. Categorical data was expressed by frequency and percentage data. Measurement data was experessed by mean±standard deviation. Count data in each group using chi-square test.When P value<0.05,the test was considered as statistically significant difference.4. Application of factor analysis, patients of diabetic nepthropathy with renal failure was analyzed on four diagnostic information, referring to "the guiding principles of Chinese medicine ".Results1. Clinical manifestations of diabetic nephropathy patients with renal failure, burnout, fatigue, dry mouth and throat, nocturia, edema, body weight difficulties, numbness, itching, lower back pain the most common embolism.2. Tongue and pulse are mostly as follows:dark tongue, tongue Red, pink tongue, thin white or yellow fur thick and greasy, slippery pulse, pulse string, or deep pulse.3. Deficiency in DN with renal failure, the yin and yang, was the most common symdrome, accounting for 70%, followed by spleen and kidney Qi Deficiency (13.75%), liver and kidney certificate (12.5%), spleen deficiency syndrome (3.75%) rare, no mere spleen and kidney qi deficiency; evidence in order to damp the most common, accounting for 82.5%, followed by water gas card (71.25%), wind Card (53.75%), blood stasis syndrome (51.25%) and wet muddy Card (50%).4. After 24 four diagnostic variables factor been analyzed, a total of 9 common factors came out, the cumulative contribution rate came to 64.027%.7 syndrome in 80 patients distributed as follows:spleen and kidney Qi and Yin Deficiency and wind permit is the most common type(23.75%), followed by the yin and yang and water and gas cards (21.25%), and the third, the yin and yang and heat, wind certificates (18.75%), which is also common. In addition, the spleen and kidney Qi and Yin Deficiency and damp, spleen and kidney Qi and Yin and water and gas card, yin and yang deficiency and blood stasis, liver and kidney followed by decreasing share certificates, were 12.3%,6.2%,1.5%,1.5%.5. Syndromes in different types of sex, age group, Mogensen IV and Mogensen V of the distribution period is not statistically significant.ConclusionsDiabetic nephropathy with renal failure syndromes are of the virtual real, actual situation mixed syndromes.Spleen and Kidney Qi and Yin Deficiency and wind card is the common syndrome.With the condition gradually worsened, sinister and yang, deficiency of both yin and yang are more common.In terms of real evil damp, water and gas cards are outstanding. |
