Objective: To study the association of HLA-DQB1 with age at onset and autoantibodies of type 1 diabetes in the Han nationality population in Sichuan province. In order to provide immune and genetic basis of screening susceptible population.Subjects and methods: 46 cases with type 1 diabetes (male 22, female 24 ), age from 2 to 58 ( M=21.5); and the patients were divided into two groups by 20 years of age at onset, <20 years 22 cases,> 20years 24 cases. The control subjects were 52 healthy individuals ( male 29, female 23 ), average age 29.6 9.7 years. Both patients and controls were the Han nationality population in Sichuan province without any blood relationships. GADA and ICA were qualitatively analysed by ELISA. HLA-DQB1 typing was performed by PCR-SSP after extracting genomic DNA by phenol/chloroform method.Results: Sensitivity of DQB 1*0201 in patients was higher than in controls (OR=18, P<0.005); but sensitivities of DQB1*0601 , *0602 in controls were higher than in patients (OR=0.07,0.31, respectively, both PO.05). No significant differences between T1DM and controls in DQB 1*03 02 and *0401 were found. Sensitivity was 43.48% for GADA, 45.65% for ICA and 69.57% for GADA and/or ICA in patients with T1DM. There were no significant differences in DQB 1*0201 and GADA between the two groups with different onset ages. The freguency of DQB 1*0602 was higher in patients with onset age>20years (P<0.05), but that of ICA and GAD and/orICA were higher in patients with onset age <20 years (both P<0.05). GADA was significantly more frequent in DQB1*0201+ patients than in DQB1*0201- patients (P<0.025). But no correlation was found between DQBl*0201 and ICA.Conclusion: Our findings show that HLA-DQB1*0201 is susceptibility to T1DM, whereas DQB1*0601, *0602 is protective in the Han nationality population in Sichuan province. DQB1*0602 may delay the onset of T1DM. ICA correlated negtively with age at onset. GADA correlated positively with DQB1*0201. PCR amplification with sequence-specific primers is an simple and rapid technique for detecting HLA-DQB1 with high specificity and sensitivity. It deserves to extend.
