| ObjectivesThe aims of this research was:(1) to explore the feasibility of treating diarrhea with the therapy of opening with acrid agents and descending with bitter agents in Singapore; (2) to analyze the mechanism of this therapy in treating diarrhea due to damp-heat; (3) to observe the clinical effects and safety of this therapy;(4) to seek the best Chinese herbs in treating diarrhea due to damp-heat in the south-east areas.Clinical DatumThis research was based on 60 cases of outpatients of the TCM clinic in Singapore. They were randomly divided into two groups. One group was the experimental group, the patients of which were treated with Changyang Xiexin Tang. The other was the control group, the patients of which were treated with Gegen Qing Lian Tang. The two groups were both made up of 30 cases. They all met the TCM diagnosis criterion.Methods(l)The herbs were all decocted with water. The patients took one dose each day. Every day two times, each time 150ml. The course was 5 days. Commonly the patients were treated only one course.(2) Clinical effects observation: single symptom: to observe the change of integrals of the main symptoms and the minor symptoms before and after the treatment. Syndromes: to observe the change of syndrome integrals before and after the treatment. Clinical effects: to observe the clinical effects and the syndrome effects after the treatment.(3) Statistical methods: the hypothesis tests in this research were all two-side tests. If p <0.05, the difference was considered significant. If p <0.01, the difference was considered very significant.ResultsAfter one course of treatment, there was significant difference in the clinical effects of the two groups according to Wilcoxon rank sum test. The experimental group had a higher curative rate than the control group. Thus the clinical effects of the experimental group were better than the control group. According to Wilcoxon rank sum test, there was very significant difference in the syndrome effects of the two groups. The curative rate and the conspicuous effect rate of the experimental group were higher than those of the control group. The experimental group was superior to the control group in improving syndromes. The effects of the experimental group showed negative correlation to patients' age, course and extent of disease. The clinical effects were worse in older, longer course and more severely ill patients.This research showed that both the experimental group and the control group had significant effects in treating diarrhea due to damp-heat. But the Changyang Xiexin Tang was better in improving symptoms, such as hot sensation in the anus, fever, thirst and nausea than Gegen Qing Lian Tang. Above all, Changyang Xiexin Tang has the function of opening with acrid agents and descending with bitter agents, clearing heat and dissolving dampness, promoting the functional activities of qi, ascending lucidity and descending turbidity, relieving bloating, regulating qi and harmonizing the middle, stopping loose bowls with fragment agents. According to pharmacology, this prescription has significant effect in diminishing inflammation and killing bacteria, alleviating pain and stopping loose bowls, clearing heat and arresting vomiting. During the research, no side effects were found. |
