Backgrounds: Avascular necrosis of the femoral head (ANFH) is a common disease in orthopaedics clinic, the main causes of the disease are hormone and alcoholic which could induce to hyperlipidemia.Apolipoproteins are the most important components in the lipid metabolisation. As the main protein of HDL, the polymorphisms of the apoAI gene (G->A substitution at position of -75 in promoter and C->T Substitution at Position of +83 in Intronl) are in close relation to the high or low apoAI and HDL-C.ApoB is the only structure protein of LDL whose gene has the most distinctive polymorphism, among which the focus of the study is the relationship between the polymorphism including loci XbaI, EcoRI, and 3'- VNTR polymorphism and serum lipid levels.ANFH belongs to "the syndrome of rheumatism" in TCM, but the phlegm is involved in the course of the disease .It had been proved the phlegm and blood stasis is in close relation to dyslipoproteinia, the higher serum lipids are the basic biochemical substances of the phlegm.Objectives: ①To investigate the relationship between the polymorphisms of apoAI, apoB gene and the syndrome of ANFH;②To investigate the relationship between the syndromes of ANFH and serum lipids metabolism;③To evaluate the impact of the polymorphism of apoAI, apoB gene on serum apolipoproteins and lipids of the syndromes of ANFH④To study the pathogenesis of ANFH induced by hormone and alcoholic⑤To evaluate the cross impact of gene polymorphism and hormone or alcoholic on serum lipid metabolism of ANFHMethods: Collecting 143 patients of ANFH and 92 Normal control. Restriction fragment length polymorphism-polymerase chain reaction (RFLP-PCR) technique was used to analyze the genotype of the ApoAI gene (-75bp in promoter and +83bp in intronl) and ApoB gene (locus EcoRI,XbaI);the VNTR polymorphism of ApoB gene was analyzed by PCR and agar gel electrophoresis;TG;CHO,HDL and LDL were examined by oxidasemethod;apoB and apoAI were examined by immune method.All the data were statistically analyzed by SAS.Results: (T)Locus -75bp polymorphism of apoAI gene is associated with the process of the syndrome of ANFH;?The process of the syndrome of ANFH is in close relationship to serum lipids metabolism, especially with higher TG, LDL and lower HDL;?Hormone and alcoholic are the main pathogenesis factors of ANFH, which have impacts on serum lipid metabolism of ANFH;Hormone mainly affects the levels of TG and ApoB, while alcoholic interfered with higher TG and lower HDL, lower ApoAI.?The polymorphism of locus -75bp in promoter of apoAI gene is related to serum lipids metabolism of ANFH, the mutation of G->A could bring to higher TG;apoB gene EcoRI polymorphism is related to serum lipids metabolism of ANFH, the mutation of E+->E- could bring to higher TG;apoB gene 3'-VNTR polymorphism is related to serum lipids metabolism of ANFH,the mutation of S->B could bring to higher TG and lower ApoA-I;?Hormone could promote the impacts of Eh—>E- mutation of apoB gene on the serum lipids metabolism of ANFH with higher ApoB;hormone interfere with the impacts of X—>X+ mutation of apoB gene to serum lipids metabolism;excessive drinking could promote the impacts of G->A mutation of apoAI gene at -75bp site on the serum lipids metabolism of ANFH with higher ApoB;excessive drinking interfere with the impacts of X-->X+ mutation of apoB gene to serum lipids metabolism with higher TG;Conclusions: ?Locus -75bp in promoter of apoAI gene is probably one of the impressionable genes of ANFH. First reported inside and abroad.? ANFH is in close relationship to dyslipoproteinemia;?Hormone and excessive alcoholic lead to ANFH both by the disorders of serum lipids metabolism?Locus -75bp polymorphism in promoter of apoAI gene is related to serum lipids metabolism of ANFH;polymorphism of EcoRI, 3'-VNTR are related to serum lipids metabolism of ANFH;the results provided the genomic evidence to the theory of dyslipoproteinemia of ANFH, and were a good example attempting to explain the phlegmand blood stasis on gene level.?The cross impact of gene polymorphism and hormone or alcoholic on the serum lipid metabolism of ANFH showed us: besides the congenital effects on the disease, postnatal factors could have important impacts on congenital effects, which help to understand the disease more deeply, benefit to prevent and cure the disease.
