How to prevent and cure septic shock is still a important problem for medicine. Research has indicate that 45%-60% sepsis are caused by the infection of Gram-negative bacteria. The component of Gram-negative bacteria's cell wall, LPS, is the major cause of endotoxic shock. Endotoxic shock is a very complicated pathologic process in pathogenesis. How to keep the balance of body's anti-inflammatory response and inflammatory reaction and prevent and cure multiple organ dysfunction have important meaning for the prevent and cure of endotoxic shock. Liver is the place to eliminate LPS. And it is the easiest injured organ among multiple organs. In recent years, some progress has been made in the research in endotoxic shock mechanism of liver damage. However, drugs with single target may not resolve the problem. Chinese herbal medicine maybe can play a multi-target role in accommodating inflammatory reaction and preventing organ function. Shenfu Injection has been widely used in the intensive care emergency, and showed good efficacy. At present, the researches in Shenfu Injection mostly focus on the protective effects in central nervous system, circulatory system, respiratory system, kidney and intestine. The protective effect in liver still lack systematic study. This experiment will investigate the protective effect of Shenfu Injection to the liver of endotoxic shock in rats, and also its possible mechanisms.1. PurposeInvestigate the protective effect of Shenfu Injection to the liver of endotoxic shock in rats, and also its possible mechanisms. Discuss the theoretical viewpoints of loss of liver toxicity network from endotoxic shock liver damage pathophysiological process and the protection mechanisms of traditional Chinese medicine.2. MethodsMale Wistar rats were randomly divided into three groups: normal, model and shenfu group. Normal group use NS to replace LPS and Shenfu Injection. Model group use intravenous injection of LPS to make rat model of endotoxic shock. model group use NS to replace Shenfu Injection. Shenfu group use intravenous injection of LPS to make rat model of endotoxic shock and inject Shenfu Injection 10 minutes after the injection of LPS. After LPS injection, four physiological recording mean arterial blood pressure begins, until 6 hours. Collect blood with procoagulant tubes and store in four degrees refrigerator. Detect rat liver function when all blood is collected. After laparotomy, get three parts of the liver. One has a 10% formalin fixation, paraffin sections and routine hematoxylin and eosin staining, and then is given histopathologic observation and image analysis. Another has 2.5% glutaraldehyde fixation and 1% tetroxide fixation, and then is given epoxy resin-embedded, ultrathin sections. After that it was given electron microscope observation. The third one is stored in refrigerator after liquid nitrogen refrigeration, and is prepared for the test of SOD and MDA.3. ResultsNormal rats'MAPs in experiments remain basically unchanged. Model rats'MAPs dropped to 46.5% of initial value when 6 hours. Shenfu rats'MAPs dropped slowly to 73.7% of initial value when 6 hours, and were significantly different vs. model group(p<0.05). Model serum ALT and AST were significantly increased vs. normal group (p<0.05). Shenfu serum ALT and AST were significantly increased vs. normal group (p<0.05), but AST decreased significantly vs. model group (p<0.05). MDA in model group was significantly increased vs. normal group (p <0.01). MDA in Shenfu group decreased significantly vs. model group (p<0.05). SOD activity in model group was significantly decreased vs. normal group (p <0.01). SOD activity in Shenfu group significantly increased (p <0.05). Light microscopy and electron microscopy observations indicate that compared with normal group, liver of model group injured seriously, liver cells of model swelling, cytoplasm osteoporosis. Liver sinus congestion and WBC incarcerated while liver of Shenfu group improved significantly.4. ConclusionShenfu injection can improve endotoxic shock rats'decreased blood pressure, and can protect the liver function, also can reduce liver lipid peroxidation. This may caused by its improvement of liver ultrastructure reduce, liver sinus congestion and WBC incarcerated, inhibition of LPS' activation of leukocyte. This can reduce inflammation, cell adhesion and infiltration, and also the release of oxygen-free radicals. This experiment provides a certain amount of scientific information to toxic damage on liver network theory.
