The Lung Cancer Lcehart Synchronous Chemotherapy Value Of Tumor Markers And Immune Function

ObjectiveTo sieve the higher saitivity and specificity tumor maker of non-small-cell lung cancer by non-operation therapy, and to research the clinical significance of TSGF and multi-tumor maker protein chip in the late-course escalated hyperfractionated accelerated radiotherapy (LCEHART) combined with chemotherapy in non-small-cell lung cancer. MethodTo collect forty-five NSCLC patiens that correspond to our qualification. All the object were treated by LCEHART (radiated by 6MV/15MV X-ray, antero-three weeks adopted normal radiotherapy 2GY/F/d,the dose of the large radiation field reached 30GY/15F/3W then adopted LCEHART, the fractionation dose of the forth week, the fifth week and the sixth week was,1.3GY/F/Bid, 1.4GY/F/Bid, 1.5GY/F/Bid, the interval was six to eight hours, the total dose reached 72GY/45F/6W, and the dose of chorda spinalis was restricted below 40GY.) combined with two cycle chemotherapy (NP program: Navelbine 25 mg/m2 , iv gtt d1,d8, DDP 25 mg/m2 , iv gtt d1-3, and treated by hydration, diuresis, prevention disgorging) .All the objects were tested TSGF and multi-tumor maker protein chip at the beginning and four weeks after therapy. According to the testing results, we sieved and analyzed the higher saitivity and specificity tumor maker of NSCLC, so as to find better tumor marker to evaluate the clinic therapy, therapeutic effect evaluation, prognosis judgement. ResultNSCLC patients were tested multi-tumor maker protein chip (CA19-9, NSE, CEA, CA242, Ft,β-HCG, AFP, f-PSA, PSA, CA125, HGH, CA153) at the beginning and three weeks after therapy. The results indicate the positive rate of CEA, Ft, CA125 is 55%, 52%, 53% in the observed objectives at the therapy beginning. It indicates significant reducing change of CEA and CA125 after therapy compared with the indexes at the therapy beginning, especially to the patients that reach CR and PR curative effect. The decreasing of CEA and CA125 keep consistence to the emerge change. About the research of life span in the near future, the result indicated significant reducing change of the six months and one year life span in CEA and CA125 positive patients.Forty-three NSCLC patients were tested TSGF examination ,the results indicate the positive rate at the therapy beginning was 65%, the sensitivity and specificity was 76% and 90% in the combination detection of TSGF, CA125,Ft,CEA. It indicated significant reducing change after therapy, and kept consistence to the emerge change. To the CR and PR patients displayed significant long six months life span in impositive TSGF patients, but the one year life span had non difference.Conclusion1. TSGF, CA125, Ft, CEA have higher sensitivity to NSCLC.2. It indicates higher sensitivity and specificity in the combination detection of TSGF,CA125,Ft,CEA.3. It indicates significant reducing change of CEA ,CA125,TSGF after therapy compared with the indexes at the therapy beginning, meanwhile the changes keep consistence to the emerge changes.4. It indicates significant reducing change of the life span in the near future to each TSGF, CA125, CEA positive patients compared with the impositive patients. So the above three tumor markers can be seemed as a prognosis indexes. ObjectiveThis research is to observe the immunity state of lung cancer by radiotherapy in sync with chemotherapy combined with pingxiao capsule, and to evaluate the effect on the immune function and the existence qualitative of pingxiao capsule in lung cancer. Accordingly to provide better guidance of clinic therapy, therapeutic effect evaluation, prognosis judgement. MethodSixty lung cancer patients were parted into two groups at random, one is simple radiotherapy in sync with chemotherapy, the other is radiotherapy in sync with chemotherapy combined with pingxiao capsule. All the two groups were carried out T-cell subgroup experiment at the beginning and four weeks after the therapy. ResultsAfter therapy CD3,CD4 and CD4/CD8 all indicates significant lower level in the group of simple radiotherapy in sync with chemotherapy (P<0.05), that denoted radiotherapy combined with chemotherapy depressed the immunity function. But in the group of radiotherapy in sync with chemotherapy combined with pingxiao capsule CD3, CD4 and CD4/CD8 all indicated higher lever after the therapy(P<0.05), especially to the CR and PR patients, that denoted pingxiao capsule improved the immunity function to lung cancer by radiotherapy in sync with chemotherapy. Meanwhile we gave KPS mark and body mass determination to all the objections at the beginning and four weeks after the therapy, it indicated the existence qulatative has been improved because of the diminish of the tumor loading after radiotherapy combined chemotherapy, pingxiao capsule can enhance the immune function of the organism ,meanwhile can improve the existence qulatative better.Conclusion1. It indicates the immunity function of lung cancer has been depressed after radiotherapy in sync with chemotherapy, but pingxiao capsule can enhance the immune function of the organism.2. It indicates the survival rate has been improved after radiotherapy combined with chemotherapy, and pingxiao capsule can improve the existence qulatative better.
