Artificial Disc Clinical Application Of Early Imaging Evaluation

The cervical disc disorder are a very common disease, especially in old adults .These kind of disease can cause great influence on peoples health.There are tow ways to treat it,in the early stage ,can chose conservative treatments, such as :cervical traction,limit cervical moving,physical therapy,acupuncture therapy and drugs.Operation is the last choice,include anterior cervical decompression and fusion and laterior decompression.The anterior cervical decompression and fusion is becom a classic surgical method,it can offer a good clinic result,but the long time follow up shows the operative method has many complications, such as :the reduce of cervical moving range ,adjacent segment degenerative.With the research of cervical artificial disc and its clinic application ,the artificial disc replacement is an effective method for surgical treatment of cervical disc disorder and it can reduce complications compare with anterior cervical decompression and fusion.The reports of its clinic research,especially film evaluation is few.In this test,one part,is the clinic research which take the cervical artificial disc replacement to treat cervical disc herniation,all cases follow up 1 year,JOA score was used to evaluate the clinic result.Then collect patients,s film information who accept cervical artificial disc replacement treatment,and collect patients,s film information who accept anterior cervical decompression and fusion,and take some mesure ,compare with this date in different groups ,have a conclusion.Part one : the clinic research of cervical artificial disc replacement to treat cervical disc disorder. Objective to investigate the clinic result of cervical artificial disc replacement in the treatment of cervical disc disorder.Method 43 cases of cervical disc herniation and 15 cases of multi-level cervical spondylotic myelopathy who accept treatment of cervical artificial disc replacement, between 2004-2006,all patients accept the Bryan disc and follow up 1 year,With the phone call,letters and outpatient service.The clinic result were evaluated through analysis the score of JOA,and colect cervical lateral view film and moving film.Result pre-operative JOA score are 8.5 , post-operative JOA score are 15.5 ,improvement rate are 88%,all clinic sympatoms are disappear.There were no subsidence of artificial discs.Conclusion The Bryan cervical disc replacement in the treatment of cervical disc disorder offer an excellent clinic outcome.Part tow The early film evluation after cervical artificial disc replacement .Objective To evluate the film chenge on patients who accept the operetive treatment of cervical artificial disc replacement .Method 20 cases of C 5/6 cervical disc herniation who accept Bryan cervical disc replacement .20 cases of C 5/6 cervical disc herniation who accept anterior cervical decompression and fusion ,all cases accept operetive in our department between 2004—2006 .Collect all cases pre-operetive and post-operetive film date include cervical lateral view film and moving film .Then mesure the cervical curvature, the cervical range of moving ,the range of adjacent segment moving. 20 cases who accept artificial disc replacement are compared pre-operetive with post-operetive. 20 cases who accept artificial disc replacement are compared with 20 cases who who accept anterior cervical decompression and fusion . All the date are analysisted with SPSS11.0 ,paired-samples t-test was used to detect the difference among the different groups.Result 1 In the 1-year follow up 20 cases of C 5/6 cervical disc herniation who accept Bryan cervical disc replacement , the cervical curvature, the cervical range of moving ,the range of adjacent segment moving,compare pre-operetive with post-operative ,P>0.05.2 compare 20 cases who accept artificial disc replacement with 20 cases who who accept anterior cervical decompression and fusion , P<0.05.Conclusion 1 The artificial disc replacement is a good choice to treat cervicial disc herniation , no change of the cervicial curvature , the moving range of cervical the range moving of adjacent segment was found in the 1-year follow up.2 There are change of the cervicial curvature ,the range moving of cervical and adjacent segment of patients who accept ACDF was found in the 1-year follow up .
