| ObjectivesTo observe the effects of sinomenine intra-articular injection on clinical symptoms, signs of knee osteoarthritis,making an initial investigation on its mechanisms,so as to provide certain basis for its clinical application in both Chinese and western medicine.Methods60 cases according with diagnosis both with western medicine and TCM were divided into Treatment group and Control group(n=30,respectively).2ml per time sinomenine intra-articular injection and hyaluronic acid were applied respectively to Treatment group and Control group for 1 time per week and consecutively 4 weeks' treatment made up 4 courses.Both groups underwent TCM symptoms scoring and efficacy evaluation after treatment,along with grading disease condition and Western Ontario and McMaster Universities Osteoarthrtis Index(WOMAC)evaluation,to investigate the extent of score improvement before and after treatment and make comparison on the therapeutic effect between groups.Results1.Effects on disease grading:Among 30 cases in Treatment group,16 cases made up the 53.33%mild severity;8 made up 30.00%moderate and 5 made up the 16.67% severe condition.And for Control group,12 out of total 30 cases took up 40.00%mild condition;10 made up 33.33%moderate and 8 for the 26.67%severe cases.Between group comparison showed no singnificant difference(P>0.05).2.On total TCM therapeutic effects:In Treatment group,condition of 3 cases were clinically controlled,rest cases showed 15 obvious efficay and 6 effectiveness as well as 6 cases noneffectiveness,the total effective rate was 80.0%,and statistical differences were observed in comparison with Control group,as the latter group had 1 case controlled,10 obvious effective,7 of efficacy and 12 ineffective(total effective rate was 60.0%)(P<0.05).3.Total TCM syndromes score:No significant differece was seen in comparison between the two groups before treatment,and significance lied in that after treatment----both scores declined obviously than that before(P<0.01).Moreover,a between groups comparison demonstrated significant difference(P<0.05).4.On clinical symptoms:There were significant differeces between the two groups on symptoms as knee joint pain,swelling and pressing pain(P<0.01);on ankylosis and flexion-extension dysfunction,it was also of significant differecne(P<0.05).The two groups showed no statistical difference in comparisons on symptoms as pain upon going up and down stairs,soreness-tired of waist and knee,squatting pain and deteriarate upon coldness(P>0.05).5.On WOMAC score:No significant difference was seen comparing the two groups before treatment on score of WOMAC pain,ankylosis,physiological function (P>0.05),while there were obvious declines in the above indexes of treatment group after treatment with significant differences(P<0.01),so did Control group(P<0.01). Statistical significance could also be observed when comparing the two groups after treatment(P<0.05).6.On WOMAC therapeutic effects:WOMAC results showed that the Treatment group had 2 controlled,16 obvious effective,7 effective and 5 without effect(total effective rate:83.33%,and for Control group,they were respectively 0 controlled,10 obvious effective,9 effective and 11 with no effect(with the total effect:60.33%). WOMAC comparison between two groups showed significant difference(P<0.05).7.On Safety:During treatment,both the liver and kidney function and blood cells analysis were in normal range.In Treatment group there were 5 cases appeared mild erythra,6 appeared local scratching with tolerance and disappeared in several minutes. 2 cases in Control group appeared endurable joint distending pain.No cases quited during treatment.ConclusionsSinomenine Injection obviously improves clinical symptoms and joint functions of KOA,functions in analgesia and anti-inflammation to KOA,along without adverse reactions and side effects,therefore is a moderately safe medicine in treating KOA. |
