| Background: The lower limb ischemia disease with high incidence, morbidity and mortality characteristics, which seriously endangers the health of patients, reduces the quality of life, is a common disabling disease in clinical. Currently application of conventional therapy has certain limitations in clinical, and we are not satisfied with its prostecdtive efficacy. In recent years the field of medicine and biology are very concerned about the stem cells in the application of angiogenesis. Stem cell transplantation in the treatment of lower extremity ischemia has been initially sure. Bone marrow -derived stem cell which has the advantages of based convenience and rich contents, has increasingly become the hot spot in basic research and clinical studies. Bone marrow mononuclear cells can differentiate, proliferate, and eventually formate a large number of new small blood vessels in the ischemic area. We use the property of mononuclear cells to treat ischemic diseases, aimed at improving the lower extremity ischemia and saving limbs.Along with the research on traditional Chinese herbs which can promote angiogenesis becomes more extensive and in-depth, It shows that Chinese herbs have a great potential in the treatment of severe ischemic disease. Chinese herbs can be an impact on the related cytokines,such as:IL (interleukin), TNF (tumor necrosis factor),CSF(colony-stimulating factor),and increase the number of mononuclear cells and immune activity, then play a role in immune activation through inducing B cell to produce antibodies , improve vascular endothelial growth factor expression, increase the number of regenerated cytokines to promote angiogenesis. Therefore, it is necessary for Chinese herbs to intervent the research of stem cell transplantation in the treatment of lower extremity ischemia.Objective: To investigate and discuss the short-term clinical efficacy of Chinese herbs (benefiting qi and promoting blood circulation) interference with autologous bone marrow of blood stem cell transplantation in treatment of ischemic disease (Including clinical symptom score, ABI index, percutaneous oxygen); to evaluate effectiveness of Chinese herbs (benefiting qi and promoting blood circulation)on intervention stem cell transplantation in the treatment of lower extremity ischemia; to provide the basis for clinical application. Method: According to statistical estimates sample content (number of cases), failure rate of 20%, select 2003-2007 hospital ischemia of the lower extremity 72 patients will be selected,patients were randomly divided into group A (stem cell therapy and traditional chinese herbs group ) and group B (stem cell therapy and western medicine group), after the mobilization of bone marrow taken from bone marrow blood CD34 + stem cells, sampling each limb of mononuclear cells to the treatment of more than 1×109, and multi-point injection of stem cells suspension in the ischemic limb muscle. The basic treatment of the two groups is the same .At the same time Group A at the same time oral Chinese herbs (benefiting qi and promoting blood circulation); Group B oral western medicine enteric-coated aspirin, pentoxifylline enteric-coated tablets; Assess the various indicators in rencent (28 days) .Results: After autologous bone marrow stem cell transplantation, the symptoms of two groups of patients such as limb pain, shiling, numbness, intermittent claudication, ulcers were alleviated; ankle brachial index, transcutaneous oxygen partial pressure in injection site are increased, no adverse reaction after treatment. There are significant differences between Chinese herbs group and the control group (clinical symptom score and ankle brachial index, transcutaneous oxygen partial pressure), Chinese herbs group is better than the control group.Conclusion: For the patients with ischemic disease of the lower extremity arteries, autologous bone marrow stem cell transplantation can relieve pain, improve ischemic symptoms. Chinese herbs (benefiting qi and promoting blood circulation) can promote the efficacy of stem cell transplantation.;can be related to increased vascular growth factor expression, and promote angiogenesis, thereby increase limb ischemia collateral circulation, improve the blood supply. In a word, it is a simple, safe and effective method of treatment. |
