Background: Clinical trial of clinical effect of oral application Supplementation with Vitamin A and E, Zinc, Selenite for neonate with infection pneumonia has been poorly studied and results are conflicting, and the relevant biochemical and immune evidence should be discovered and discussed. Aims & methods: To assess the clinical efficacy and discover biochemical and immune evidence of vitamin A and E, Zinc, Selenite administered by mouth for neonatal pneumonia. we investigated the effect and discovered the relevant biochemical and immune evidence of a single orally 4500IU vitamin A daily(VA group), the orally 4500IU vitamin A and 20mg vitamin E, 9mg zinc,20μg selenite daily (multinutrient group), and orally placebo-controlled normal sodium 1ml daily(placebo group) in 111 neonates in the randomized, double-blind, placebo-controlled, clinical Trial for 7 days treatment.Results: The symptom recovery times are shorten on VA group and multinutrient group (p<0.01), but there is not significantly different between these groups (p>0.05). Serum RBP is not significantly different on VA group (p>0.05) and multinutrient group (p>0.05) after 7 days treatment, but it is higher on placebo group (p<0.01). Serum VA are higher on VA group and multinutrient group (p<0.01) after 7 days treatment, does not differ significantly on placebo group (p>0.05). Serum IgM MDA are lower on VA group and multinutrient group (p<0.05) after 7 days treatment, did not differ significantly on placebo groups (p>0.05). The changes of Serum RBP VA IgM MDA are not significantly different between VA group and multinutrient group (p>0.05)。No toxic symptoms are observed in period of hospitalization. .O Conclusion:These daily low-dose oral nutrient treatment significantly promote the clinical symptom recovery, enhance the immune function and antioxidation capacity, but single oral vitamin A doesn't play different efficacy with oral vitamin A, E, Znic and Selenite, the time of supplementation may be too short to observe the significant difference. The oral multinutrient administration is safe and feasible for neonate on clinical treatment.PART2 MULTIVARIATE ANALYSIS OF THE INFLUENTIAL FACTORS IN MULTINUTRIENT TREATMENT FOR NEONATE WITH INFECTION PNEUMONIAObjective: To explore the influence of background data in multinutrient treatment for neonate with infection pneumonia. To study the relative risk factors in VA nutritional state. Methods: the orally vitamin A and vitamin E, zinc, selenite daily in 111 neonates in the randomized, double-blind, placebo-controlled, clinical Trial for 7 days treatment. We investigated and analyze the relevant biochemical, immune evidence, clinic data and background data, with multivariate stepwise regression analysis.Results: Mother's age, parity, gestation age and birth weight play positive influence to clinical restoring. Neonate's age and birth weight play a positive role to serum IgM in early stage of pneumonia, but mother's age is negativity. Neonate's age is positivity in enhancing serum VA, and is negativity in advancing serum IgM. Mother's age play a negative role to heighten serum RBP.Conclusion : Mother's age, parity, gestation age and neonate's age is important factors to influence the clinical treatment effect. Great age and many parity mother's neonate, earlier period and large for date infant are more possible to VAD during sick period, and are more slow to recover. So they need the nutritional intervention all the better. PART3 THE EFFECTS OF LOW-DOSE VITAMINS A AND E, ZINC, SELENITE ON ANTIOXIDANT CAPACITY IN NEONATE WITH INFECTION PNEUMONIAAims & methods: To assess the changes of Antioxidant marks of a single orally 4500IU vitamin A daily(VA group), the orally 4500IU vitamin A and 20mg vitamin E, 9mg zinc, 20μg selenite daily (multinutrient group), and orally placebo-controlled normal sodium 1ml daily(placebo group) in 65 neonates in the randomized, double-blind, placebo-controlled, clinical Trial for 7 days treatment.Results: Serum RBP is not significantly different on VA group (p>0.05) and multinutrient group(p>0.05)after 7 days treatment, but it is higher on placebo group(p<0.01). Serum VA is higher on VA group and multinutrient group (p<0.01) after 7 days treatment, does not differ significantly on placebo group(p>0.05). Serum MDA is lower on VA group and multinutrient group(p<0.05) after 7 days treatment, did not differ significantly on placebo groups (p>0.05). Serum T-SOD is lower on three groups (p<0.01) after 7 days treatment. Serum GSH-Px is higher on VA group (p<0.05) and multinutrient group (p<0.01) after 7 days treatment, did not differ significantly on placebo groups (p>0.05). The changes of Serum RBP VA MDA T-SOD are not significantly different between VA group and multinutrient group (p>0.05), but the change of Serum GSH-Px is higher in VA group than multinutrient group. Conclusion:These daily low-dose oral nutrient treatment significantly promote the antioxidant capacity , especial in preterm infant, low birth weight infant, great age woman's neonates. The effect of oral vitamin A, E, Znic and Selenite is better than single oral vitamin A.PART4 THE EFFECTS OF LOW-DOSE VITAMINS A AND E, ZINC, SELENITE ON IMMUNE FUNCTION IN NEONATE WITH INFECTION PNEUMONIAAims & methods: To assess the changes of immunological marks of a single orally 4500IU vitamin A daily(VA group), the orally 4500IU vitamin A and 20mg vitamin E, 9mg zinc, 20μg selenite daily (multinutrient group), and orally placebo-controlled normal sodium 1ml daily(placebo group) in 111 neonates in the randomized, double-blind, placebo-controlled, clinical Trial for 7 days treatment.Results: Serum IgM is lower on VA group and multinutrient group (p<0.05) after 7 days treatment, did not differ significantly on placebo group (p>0.05). Serum SIL-2R is higher on VA group (p<0.05) and multinutrient group (p<0.01) after 7 days treatment, did not differ significantly on placebo group (p>0.05). Serum IL-6 is lower on VA group and placebo group (p<0.01) after 7 days treatment, did not differ significantly on placebo group (p>0.05). The changes of Serum IgM and SIL-2R are not significantly different between VA group and multinutrient group (p>0.05).Conclusion:Suppression of immune function was observed in in preterm infant, low birth weight infant, great age woman's neonates.These daily low-dose oral nutrient treatment is more benefit in mononuclear phagocyte system significantly, the intervention can suppress the execessive immunologic injury during restoration stage.
