| Objective To investigate the clinical value of plasma level of nitrogen terminal brain natriuretic peptide(NT-proBNP) in the old patients with non-typical heart failure.Methods 60 patients were randomly choosed.They received NT-proBNP test at entry.And echocardiography examination was performed at the same time to measure left ventricular rejection fraction,left ventricular end-diastolic diameter, endocardial fractional shortening,left atrial diameter.Heart failure and non-heart failure group were divided by left ventricular rejection fraction.A statistical analysis was used to analyze the relationship between these items.Result The levels of NT-BNP were significantly different between chronic heart failure(CHF) group and non-CHF group(t=13.19,P<0.001).Moreover, elevated NT-proBNP levels displayed a higher sensitivity and specificity in dignosinging heart failure than left ventricle ejection fraction(LVEF)(u=4.22, P<0.01).Plasma levels of BNP were a negative correlation with left ventricular rejection fraction(r= - 0.543,P<0.001) and endocardial fractional shortening(r= -0.457,P<0.001).Plasma levels of NT-proBNP were a positive correlation with left atrial diameter(r=0.387,P<0.001) and left ventricular end-diastolic diameter (r=0.421,P<0.001).There was no significant difference between male and female CHF patients.Conclusion NT-proBNP is a quantitative index in the diagnosis of CHF. Compared with UCG,NT-proBNP showed higher sensitivity and negative predictive value in diagnosing CHF.And it could better evaluate cardiac structure and function in patients with congestive heart failure. |
