BackgroundNosocomial Infection is still one of the main influence factors which threaten patients' safety in hospital. Sterilization and isolation are important measures in preventing and controlling nosocomial infection. The first step of controlling nosocomial infection is breakthrough in disinfection and sterilization. Significan nosocomial infection emergent events caused by misstep in disinfection and sterilization resulted in substantial losses for people healthy were reported often. Therefore, emphasis of sterilization isolation system in medical workers and education of health knowledge in inpatients during the period of treatment and nursing may decrease the hospital infection. With the late beginning and rapid progress of relative reserch and work in our country, the disinfection and sterilization of hospital has gradually become legalization and standardization. However, literature search suggests that there are still lots of problems in disinfection management practice. The complexity and diversity of nosocomial infection pathogen make disinfection management in hospital face enormous challenges.ObjectivesThis study was designed to probe into the status quo of the disinfection management of public hospitals in Minhang District, to analyze the disinfection efficacy of key clinical departments, to explore the problems of disinfection management of public hospitals and accordingly to put forward the relevant suggestions which provide policy recommendations.MethodsThe research data were derived from field survey, disinfection examination and focus group interview. Nine public hospitals in Minhang District were selected as the investigation field, including six community hospital and three second-tier hospital. The data were put into database through Epidata 3.0, and processed and analyzed mainly using descriptive statistics through statistical softwares of SPSS 11.5.Results1. The status of basic configuration of disinfection work of the investigated objects: (1) Both community hospital and second-tier hospital have established hospital nosocomial disinfection management and sterilization isolation system. (2) Both community hospital and second-tier hospital have scored certain achievements in infrastructure construction and propaganda of sterilization isolation work.2. The status of disinfection and sterilization monitoring effect of the investigated objects: some community hospital has not developed monitoring of chlorine disinfection in key clinical departments yet, and all second-tier hospital has not developed monitoring of glutaraldehyde concentration in endoscopic ventricular yet. Some community hospital and second-tier hospital has not developed monitoring of amount of bacterium contamination of the disinfectant.3. The status of disinfection management of key clinical departments of the investigated objects: (1) The usage rate of Contagious Diseases outpatient department of was low, and the recognition degree showed seasonal tendency. (2) Establishment of standardization in key clinical departments was inadequate. (3) Individual protection of medical staff was poor, harmful for formation of nosocomial infection bi-directional protection. (4) Measures in disinfection controlling existed occupational hazards.Conclusions and Policy Recommendation1. Administrators of community hospital and second-tier hospital should pay more attention to the establishment and implement of relative management system and the significance of the disinfection efficacy of key clinical departments, emphasize the establishment of standardization, and improve basis equipment of disinfection and sterilization and individual protection of medical staff under the condition of limited fund, Reinforce disinfection and sterilization effects monitoring, further improve medical service personnel protection consciousness.2. Supervision Department should fulfill the supervise function, guide hospital establish standardization under law, expand propagandizing channels actively, increase patients' understanding of nosocomial infection, implement external social supervision, enhance daily supervision and spot-check of outpatient department of infectious diseases, put an end to seasonal tendency, reinforce technique support.
