BACKGROUND AND OBJECTIVE: Rational use of perioperative antibiotics can reduce the incidence of postoperative infection and mortality, shorten the patient's hospitalization time. The abuse of antibiotics has increased the incidence of nosocomial infection. It is found that the rational usage of perioperative antibiotics is low according to the application surveys of perioperative antibiotics. In order to regulate the rational use of antibiotics, many countries have formulated guidelines for the use of antibacterial drugs. China has also issued the national guideline on the clinical use of antibacterial medicines. The comparative study of the application of antibiotics before and after the publishing of the guidelines has been rarely reported. The use of perioperative antibiotics in the situation of general surgery and emergency surgery are summarized and the reasonable of the antibiotics use are evaluated by retrospective cohort study method. The risk factors on irrational use are analyzed by multivariate analysis method. The situation of antibiotics use before and after the publishing of the guidelines is also discussed. This paper provides a reference for decision of emphasis of management and rational use of antibiotics in clinic. METHODS: The eligible cases were collected. The situation, laboratory examination data, process of surgery, program of antibiotics use and clinical outcome of treatment of patients were recorded. These data were classified and statistical analyzed. According to the results, the emphasis of management of perioperative antibiotics was defined.RESULTS: The relationship of the status of antibiotics use and irrational use showed that course of treatment, adverse events and drug treatment time were high risk factors of irrational use. Usage and susceptibility test could decrease the risk of irrational use. The results of the comparative studies showed that course of treatment was significantly shortened, attention-degree of doctors on susceptibility test was increased. In general the rate of rational use of antibiotics was increased.CONCLUSION: There are many problems on irrational use of antibiotics. The promulgation of the national guideline on the clinical use of antibacterial medicines can improve rational use of antibiotics. In order to realize the rational use of antimicrobial drugs, the full cooperation of the government, hospitals, drug manufacturers and patients are needed. It is a long-term and arduous task.
