| Objective: To analyze public hospital managerial personnel's willingnes of reform policies through surveys of public hospital personnel. To gauge the impact managerial personnel expect reform to have on themselves, through the management personnel's satisfaction survey. To analyze the factors influencing managerial personnel's willingnes of public hospital reforms. And to make targeted policy recommendations that will facilitate the smooth implementation of reform of public hospitals.Methods: The study applies document analysis, analyzes the related research on medical system reform from domestic and international sources, and uses the Delphi method to create the survey for to the pilot hospitals and the other hospitals in Anhui province. To analyze the date t-tests and chi-square tests will be used, and logistic regression analysis will be used to find the factors influencing reform of the management personnel so as to finally summarize the policy recommendations and to ensure a smooth start the reform of public hospitals.Results: (1) A summary of the public hospital management personnel: middle management accounts for 39.46% of the managerial staff; hospital administrators 13.0%; Personnel without titles accounts for 14.35%; primary title personnel account for 21.97%. Persons less than 30 years old account for 17.49%; persons aged 30 to 40 27.80%; Degree levels of hopsital personel: 41.26% have bachelor degree 55.61%; professional management personnel account for 24.66% of the staff, medical professionals 36.32% of the staff; 23.31% of the management personnel take the clinical as a part-time job. (2)The contrast analysis of management reform intends between the pilot city management personnel and the other groups of personnel: management personnel from pilot city and the other cities have significant differences. For example: "Do you agree that public hospitals should support grassroots health development?" (χ~ 2 = 8.461, P = 0.037); "Do you agree that the hospital should promote medical resources rationally through cooperation and restructuring" (χ~ 2=14.474,P= 0.006); " Do you agree there should be comprehensive evaluation, incentive and restraint mechanisms for the hospital director " (χ~ 2= 7.952, 0.04).(3) Satisfaction analysis of public hospital management personnel with their hospitals: chi-square analysis results of satisfaction survey between the management employees in pilot city and in other cities, "do you think that the hospital departments and posts must have clear divisions"? Only in this question do we find significant difference (χ~ 2 = 11.17, P = 0.025). Response to the other nine questions results show no significant differences.(4) Managerial personnel's understanding about reform policy need to be further improved and their concerns on it addressed. Management personnel already has a basic understanding of two recent key documents on medical reform, but more efforts need to be put in effectively making hospital management aware of the intended reforms.(5)Factor analysis of the reforms intended for public hospital management was found using logistic regression analysis. This study finds five factors that influence the awareness of and opinions on the reforms intended for public hospital management. They are policy awareness, personal income, personal career development, satisfaction with the hospital, and concern the head of hospital management shows for the reforms.(6) Because of managerial personnel's high job satisfaction and uncertainty about the impact of the reforms, they have begun to display some path dependency.Conclusion: Studies show that the sample of medical reform hospital administrators know the extent of the relevant policies to be further enhanced,the change resistance from the internal management needs to be eliminate,finally,the recommendations for improvement been proposed from the macro and the micro level,, the survey provides a basis for decision making to promote the health reform policy for the government departments.The improve part for this case study: Shortcomings of this study is that:the researchers did not done the further more analyze Because of financial constraints topics,the management reform willlingnees the degree of influence in public hospitals reform for further quantitative analysis is not. At the meantime, this program is relating to huge number of different area, including government, supplier and demander. One of the most important disadvantage of this case study is that it was not provided the research of supplier innovate desirability from demand and government side, so it causes the evidence in the case study is not strong enough, also it leads to the new direction of the case study in future. |
