The Theory On Differentiation Of Disease And Symptoms And Signs Of Constipation With PD And Its Clinical Research

Objective To inquire into the syndromes and the distribution characteristics of syndromes factors of Parkinson's disease with constipation, to reveal the inner pathogenesis of Parkinson's disease with constipation; to explore relevant independent clinically risk factors which induce Parkinson's disease with severity constipation.Methods Collected patients who were inpatients or outpatients of Nanjing Hospital of TCM and Nanjing Brain Hospital. We formulated syndromes factors standards and dialectical scale by referencing Parkinson's disease and the TCM syndrome type standard of functional constipation, and combining with related literature review.We respectively studied the distribution regulation of syndromes in Parkinson's disease and constipation. We evaluated the Parkinson's disease using Parkinson's disease clinical evaluation related scale,such as unified Parkinson's disease rating scale (UPDRSIIK H&Y),Parkinson's disease symptoms scale (non-exercise activity PDNMS) and associated with Parkinson's disease psychological scale (HAMD HAMA).Using chronic functional constipation Cleveland scale (CCS) and constipation the quality of life of the patients scale (PAC-QoL) to evalute the clinical constipation syndroms of Parkinson's disease with constipation. Parkinson's drug using are to levodopa equivalent dose (LED) conversion. Using multivariate linear correlation and Logistic regression analysis method, discussed the related and independent risk factor between Parkinson's disease with constipation.Results The syndrome distribution of Parkinson's disease is that the type of Deficiency of both Yin and Yang is the major part in constipation group and the type of Deficiency of the Liver and Kidney is major in nonconstipation group; The syndrome distribution of constipation:the type of Deficiency of the Spleen-Yang and Kidney-Yang is major in Parkinson's disease, the type of Deficiency of both Qi and Yin is major in functional constipation; the type of Deficiency of both Yin and Yang which is much more than any other types in the syndrome of Parkinson's disease which are Parkinson's disease with severity constipation,but constipation syndrome types is mainly the type of Defici ency of the Spleen-Yang and Kidney-Yang. The Deficiency of Yang is the common syndrome factor in Parkinson's disease with severity constipation; the factor of location is spleen and kidney. Parkinson's disease with constipation is more serious than functional constipation. The severity of Parkinson's disease with constipation is only correlate to the symptoms of non-exercise activity. The independent risk factor is "the digestive system symptoms".Conclusion The syndromes distribution regulation of Paronkinson's disease with constipation can respectively treat Parkinson's disease and constipation as a breakthrough to a single disease differentiation dialectical studies. Parkinson's disease with constipation have a seriously influence in the quality of life of the patients who have Parkinson's disease,so we should pay more attition to the severity of constipation.
