| [Objective]With the development of rapid life, increased work pressure, serious environmental pollution, diet and lifestyle changes and the changes in people's attitudes generational fertility, infertile couples,account for 10% to 15%married couples in reproductive age. Recent studies have also found that the incidence of infertility rises affected by the environmental pollution and unhealthy living habits. The highest local infertility rate is 24.3%,and the lowest is 0.8% in China.Infertility not only brings patients physical pain, but also does great psychological harm to patients. Infertility as a psychosomatic disease, seriously affecting the quality of life of patients, has become an important medical and social problem. With the increased incidence, we should pay more attention. Biological factors on female infertility are researched more, but environmental and psychological factors are less comprehensive studied. Therefore, this objective of study is to explore the impact of environmental factors on female infertility.The result which will be contribute to provide scientific basis for prevention and control femeale infertility and provide scientific basis.[The subjects and methods]1. Through method of the questionnaire survey, adopting the basis of 1:1 paired hospital case-control study, the outpatient of affiliated Qilu hospital of shandong university and provincial hospital reproductive center in July 2010~September 2010 diagnosed as infertile women were confirmed as case group, and the famale outpatient of other departments of the same period no serious medical problems of female infertility patients were confirmed as control group. Each case had a comparison, inclding the matching variables for national, age (±2 years).2. This content of this study is:the generation of the issued and her spouse (sex, age,occupation,education,family finances,etc.), personal medical history and medication history (whether suffering from chronic diseases, such as diabetes, tuberculosis, radiation therapy, or chemotherapy, surgery, history of abortion,birth control pills, diet pills, hormone drugs, etc.), menstrual history and marital sexual history (age at menarche age, sex, age, history of abortion), dietary habits and lifestyle habits (picky eating, smoking, drinking,etc.), family environment and occupational environment (whether contacts ionizing radiation, housing decoration, high temperature, night shift, etc.), mental health and interpersonal relationships (husband-and-wife relationship, peer relationships, mental pressure, a significant negative life events etc.). All exposure information was before diagnosis.3. Applied Epidata3.02 establish database and entry with double dual verification methods, used SPSS 16.0 software to infertile women (patient) basic descriptive analysis,with the test for statistical hypothesis testing and combined with professional knowledge. To make the factors that may affect female infertility univariate analysis, initially screened statistically linked factors and professional related factors. On the basis of using COX model of SPSS 16.0 to make conditional Logistic regression analysis, fited the factors to analyze and the factors to adjust with multivariate conditional logistic regression model (Conditions Logistic enter level=0.1, eliminate level=0.15), removed the confounding factors to analyze the risk factors or more independent protective factors.[Results]This survey was awarded questionnaire 520 copies, and valid questionnaires are 494 copies, effective response rate of the questionnaire was 95%. In all case 247 cases were female infertility, and 247 cases are normal fertility. The results were as follows:(1) Univariate analysis found that:female infertility was related to residence, occupation, cultural degree, the occupation and cultural degree of spouse, history of radiation therapy, gynecological surgery, gynecological inflammation, thyroid hormone-related in treatment history and disease in medical history;And was related to menstruation,dysmenorrhea, ectopic pregnancy;And was related to eating pickled foods fried foods,alcohol consumption,smoking,staying up late, perming-related of personal living habit and habit in diet;And was related to housing decoration, contacting with heavy metals, exposure to radiation,exposure to pesticides,hot work of family environment and occupational environment;And was related to colleague relation in in-laws, stress,uneasy,Helpless despair,the way to solve a bad mood of mental health and interpersonal relations.(2)Multiariate analysis showed that gynecological inflammation (OR=2.048), menstrual rule (OR=5.846), other performance-enhancing drugs (OR=32.971), staying up late (OR=2.937), housing decoration (OR=2.963), exposure to radiation (OR=2.506), contacting pesticides (OR=2.908) and mental pressure (OR=4.101) were the main environmental factors.[Conclusions]Female infertility caused by several factors:Female infertility was caused by many factors, environmental factors, gynecological inflammation, menstrual regularity, hormone drugs, staying up late, housing renovation, exposed to radiation, exposure to pesticides and stress play an important role. In this study, the same as other case-control study which existed memories bias and small sample size, waiting to increase sample size to further discuss the influence factors of female sterility. |
