Objective The aim of this study is to research the rehabilitation mechanism of six-character formula respiratory gymnastics on pulmonary function based on the principle of strengthening earth to generate mental on airway resistance and respiratory muscle in patients with chronic obstructive pulmonary disease. It is not only proving objective evidence for choosing six-character formula respiratory gymnastics to prevent and cure pulmonary function, but also working the academic base for seeking an effective, reasonable rehabilitation for design and verification.Method sixty-three COPD patients on stable phase collected from Fujian Provincial Hospital and The People's Hospital of Fujian Province were recruited in the clinical trail. Three patients were exited the experiment (test group lost two samples; control group lost one samples). At last sixty enrolled patients were randomly divided into test group (thirty samples) and control group(thirty samples). The patients in both groups were given routine treatment and health education, while those in the test group were given six-character formula respiratory gymnastics training and those in the control group were given whole body respiratory gymnastics training. All patients should take training for three months,30 minutes per time every day. All patients should fill the training time and feelings in the form after exercise. During the experiment, patients will receive regular follow-up through telephone or home visit by trainers. The index of forced expiratory volume in one second (FEV1), FEV1%pred, forced expiratory volume in one second/forced vital capacity (FEV1/FVC), airway resistance(Raw),specific airway conductance(sGaw) and maximum inspiratory pressure(MIP), maximum expiratory pressure(MEP) were evaluated before and after intervention.Result1. Pulmonary functionIt was revealed by pulmonary function test that significant difference was only found in FEV1 (P<0.01) in both of the two group compared with pre-rehabilitation[test group(FEV11.12±0.33;1.30±0.32 P<0.01)];control group(FEV11.28±0.36;1.30±0.35 P<0.01), while there were no significant differences in FEV1%pred and FEV1/FVC (P>0.05). There were no significant differences between two groups (P>0.05).2. Airway resistanceThere was significant increase in sGaw in test group[26.00 (20.00~39.75); 36.00 (22.25~64.75)P<0.05],but Raw in test group has no significant difference (P<0.05). But Raw, sGaw of patients in control group has no significant difference (P>0.05). The sGaw after the training of six-character formula respiratory gymnastics was significantly larger compared with whole body respiratory gymnastics (test group42.07±23.62; control group31.47±16.08 P<0.05), There was no significant difference in Raw between two groups (P>0.05).3.Respiratory muscleAfter three months training, the MIP, MEP of test group has no significant increase (p>0.05), and also no difference in control group (P>0.05). There were no significant difference between two groups (P>0.05).4. Correlation analysis between pulmonary function and respiratory muscleThey were significant correlations both between FEV1,FEV1%pred,FEV1/FVC and MIP(r=0.581,0.472,0.403 P<0.05);while there were correlation between FEV1/FVC and MEP(r=0.464 P<0.05).5. Correlation analysis between pulmonary function and airway resistanceThe were significant correlations both between FEV1,FEV1%pred,FEV1/FVC and Raw, sGaw[Raw(r=-0.658,0.579,0.486P<0.01);sGaw(r=0.707,0.678,0.547 P<0.01)].6. Multiple regression among pulmonary function and airway resistance, respiratory muscleMultiple regression model analysis revealed that pulmonary function(FEV1, FEV1%pred, FEV1/FVC)were dependent variables while respiratory muscle(MIP,MEP), airway resistance(Raw,sGaw)were independent variables. The change of FEV1 is affected by MIP, Raw, sGaw[MIP(β= 0.322 P<0.05) Raw(β=-0.399 P<0.05) sGaw(p= 0.432 P<0.01)];The change of FEV1%pred was influenced by MIP,Raw,sGaw [MIP(β=0.118 P<0.05) Raw(β=-0.018 P<0.05) sGaw(P=0.208P<0.01)];The change of FEV1/FVC was effected by MIP, MEP, Raw, sGaw [MIP(p=0.062 P<0.05) MEP(P=0.072 P<0.05) Raw(β=-0.005 P<0.05) sGaw(p= 0.429 P<0,01)].Conclusion Airway resistance takes benefit from six-character formula respiratory gymnastics; Pulmonary function is affected by respiratory muscle and airway resistance. The rehabilitation mechanism of six-character formula respiratory gymnastics on pulmonary function based on the principle of strengthening earth to generate mental on respiratory muscle and airway resistance in patients with chronic obstructive pulmonary disease.
