Objective:To compare the therapeutic effect of patient-controlled femoral nerve analgesia(PCFNA) and patient-controlled epidural analgesia(PCEA) on postoperative pain, side effects and functional rehabilitation after total knee arthmplasty(TKA), To evaluate analgetic the efficacy, safety, and makes further discussion on knee function recovered without help.Methods:Forty-six patients (ASAâ… -â…¢)undergoing unilateral TKA were randomly divided into group PCFNA and group PCEA. All patients received unilateral spinal anesthesia. VAS pain scores were recorded at each time point of 4,8,12,24and 48h. Bromage scores was recorded on exercise block rating at each time point of 4,8,12,24and 48h. Ramsay scores was recorded on calm degree at each time point of 4,8,12,24and 48h. Whether have sleep disorders after 3 days. And the rotation were recorded at the time point of 1d,2d,3d,1w,2w,1m,3m. Record after 48 hours cases of side effects and within 48 hours after patient use related drugs:stop vomit medicine, nonsteroidal anti-inflammatory drugs and opiates drugsResults:No statics significance was found between the VAS pain score. No statics significance was found between Ramsay scores and Sleep quality. The Bromage scores in group PCFNA were obviously lower than those in group PCEA and the early functional rehabilitation were better than the group PCEA, no statics significance was found on long-term functional recovery. The side effect in group PCFNA were obviously lower than those in group PCEA.Conclusion:PCFNA provides effective pain relief after total knee arthroplasty. the early functional rehabilitation PCFNA is better than group PCEA. There were no significant differences in effects of long-term functional recovery. After total knee arthroplasty, PCFNA causes fewer side effects and more catheter-related problems than PCEA.
