| ObjectiveEvaluate the short tern & long term results; safety and affected factors of curing Menorrhagia by using Uterine Balloon Thermo Ablation,UBT.MethodReview and analysis 122 Menorrhagia patients from 2001 to 2005 that refused to take hormone therapy ( including the cases had little effect by taking hormone theropy) and/or excised womb through operation, and have no needs to breed. Make UBT won't be a contraindication. Investigate and record the menstrual period, menstrual blood volume, premenstrual syndrome, dysmenorrheal, sexual life and postoperative complications etc. Analysis and evaluate the affected factors of curing and other comments about this technology. The results were analyzed by one-factor analysis of variance with SPSS16.0 statistical software. The days of menses before and after treatment were compared by using pair t-test. Influence factors of using the homogeneity of variance test,χ2test and rank sum test. Significance was set at P<0.05.ResultWe have investigated 122 cases in total, finished the investigations after 6 months 1 year, 2 years, 3 years, and 5 years.1. The rates of amenorrhoea are 46.72%,50.82%,40.98%,46.72%,48.36%;2. The according efficiency are 100.00%,99.18%,92.62%,91.80%,87.70%.3. The according days of menses: :(1.81±2.14)d,(1.62±2.10)d,(2.22±2.29)d,(2.19±2.13)d,(1.89±2.51)d.4. The efficiency of curing dysmenorrhea, breast tenderness, abdominal pain, fall, lower limb edema, and irritability / depression after treatment are 62.16%,22.7%,88.2%,88.9%,75.0%.5. 112 patients, There are 28 cases with adenomyosis, 6 cases due to recurrence of menorrhagia and / or increased dysmenorrhea, had hysterectomy after UBT after 2 to 5 years. 2 patients had uterine fibroids increased significantly due to uterine fibroids after 3 to 5 years, compression symptoms occur gradually. They have done myomectomy and uterine subtotal resection already. Followed up for 5 years, 7 patients due to recurrence of menorrhagia and accept the second UBT treatment. The further surgical intervention rate is 12.30% after UBT treatment.6. 88 patients (72.13%) return to normal sexual life after 1 to 3 months, 72 patients (81.8%) thought that the qualities of sexual life are improving than before.7. There is no severe abdominal pain and no complications occurred during UBT treatment.8. No occurrence of endometrial cancer patients and pregnancy patients are found 5-year follow-up after treatment.Conclusion1. UBT has an exact improvement on curing menorrhagia in short tern, and stable result in long tern.2. UBT has lots of advantages on curing meorrhagia:tiny trauma, operation time is short,simple procedure, recovery is fast, can be in the operation without using anesthetic. The operation can be done in the outpatient service completely.3. The patients with dysfunctional uterine bleeding, endometrial polyps has definite effect. The patients with adenomyosis,uterine myoma though had a effective treatment effect, but further study of the influence of efficacy remains to be will be needed in the future.4. UBT treatment and after treatment have no significant postoperatie complications, the risk of endometrial cancer will not be increased either. |
