Clinical Pathway Applied Research Of The Patients About The Lubar Intervertebral Disc Protrusion MED Operation

Objectives:On the basis of refered to a great quantity references, seeked advice from the specialist, and mirrored domestic experience, the research carryed out the clinical pathway in the lubar intervertebral disc protrusion patients and who accepted the Microendoscopic Discectomy. To appraise the effectiveness of the clinical pathway in the MED operation, through compared the average hospital day,the average preoperative hospital day,the average postoperative hospital day,the average cost of hospitalization,Operative incision the first healing rate,recovery rate and patients' degree of satisfaction.Methods:Searched the MED operation patients 42 from the January to December in the year 2009, screening 35 patients to the control group, collected 30 patients of the group of clinical pathway from the January of 2010 to the March of 2011. Contrastived and analysised the the average hospital day,the average cost of hospitalization,the average preoperative hospital day,recovery rate,Operative incision the first healing rate,recovery rate and patients' degree of satisfaction of two groups.Results:(1) The average hospital day of clinical pathway group was 13.83±3.25 days, the control group was 16.14±4.93 days; the average preoperative hospital day of clinical pathway group was 5.77±2.54 days, the control group was 6.14±3.26 days; the average postoperative hospital day of clinical pathway group was 8.3±2.7 days, the control group was10±3.66 days; the disparation of the average hospital day and the average postoperative hospital day between the two groups had statistical significance(P<0.05), the clinical pathway group' disparation of the average hospital day and the average postoperative hospital day were shorter than the control group; (2) the average cost of hospitalization of clinical pathway group was 9678.79±1777.86 yuan, the control group was 11630.03±5069.55 yuan;, the disparation of the average cost of hospitalization between the two groups had statistical significance(P<0.05), the clinical pathway group' disparation of the average cost of hospitalization was lower than the control group; the disparation of the inspection fee and the berth fee disparation had statistical significance(P<0.05),but the western medicine fee,operation fee,therapy fee and the others had no significant difference (P>0.05);(3)the recovery rate and Operative incision the first healing rate between the two groups had no significant difference (P>0.05);(4) the patients' degree of satisfaction of clinical pathway group was 91.5±5.92, the control group was 87.23±4.43, P=0.002, P<0.05, the disparation had statistical significance(P<0.05).Conclusions:(1) Enforced clinical pathway could to shorten the hospital day of the lubar intervertebral disc protrusion patients and who accepted the Microendoscopic Discectomy in force; (2) Enforced clinical pathway could reducd the cost of hospitalization, mainly was the inspection fee and the berth fee; (3) made the flowsheet of Medical treatment become unhindered,ensured the Medical treatment quality; (4) to promote the communication between the Medical Staff and the patients, exaltation the patients'degree of satisfaction. The research effectively advanced to decreasd the hospital day and cost of hospitalization, increased the patients' degree of satisfaction and Medical treatment quality. Therefore, the research has far-reaching perspective to Hospitals' quality management, and has far-reaching meaning for the further innovation of Medical Therapy Unit and increase the Medical treatment quality.
