| ObjectivesThis paper synthetically analyzes the assistance input-output efficiency about advanced chistosomiasis in Jingzhou's designated hospitals, the influencing factors of hospitalization expenses and quality of life on advanced schistosomiasis patients, and the effect and benefit of assistance policy. Based on above works, to discover the questions existing in assistance work about advanced schistosomaisis, the perfect related policies of advanced schistosomiasis assistance, reasonably allocation of limited health resources and the optimization and improvement of management level of personal advanced schistosomaisis in assistance work.ContentsThe main content of evaluation on medical care of advanced schistosomaisis from the standpoint of service providers is as follow:1) Basic conditions of designated hospitals in Jinzhou (scale, character, manpower, financial resources and working details during 2006-2008);2) Evaluation of input-output efficiency (understanding the evolution from the time series analysis);3) Expenses of advanced schistosomiasis patients in designated hospitals (identifying controllable and uncontrollable influencing factors of hospitalization expenses).The main content of evaluation on medical care of advanced schistosomaisis from the standpoint of service demander is as follow:1) Life qualities of patients after therapy;2) Patient's degree of satisfaction on the institution;3) Evaluation of the policies by patients and evolution of life qualities of them.MethodsThe data was collected through survey study and literature research. Epidata 3.02, SPSS17.0 and Deap 2.1 were employed in this study. Descriptive analysis, the method of data envelopment analysis, multivariate analysis and the new gray correlation analysis were conducted to analyzing the basic work condition of eight designated hospitals of assistance on advanced schistosomaisis between year 2006 and 2008, resource input-output efficiency of designated hospitals, constitute of the hospitalization expenses and its influencing factors comprehensively. The quality of life of advanced schistosomaisis patients after receiving assistance were evaluated by the EQ-5D scale and quantitatiy interview.Results1) Seven hospitals were set up specifically beds for treating advanced schistomiasis patients, the average specifically beds for advanced schistomiasis patient accounts for 72.7% of the total number of beds. Special appropriation on assistance on advanced schistomiasis patients in Jingzhou is increased by 15.0% in 2008 than in 2006, and the total number of patients by 3.68%.2) The DEA results of data of 2006 to 2008 suggested that the comprehensive effective hospitals were 4, 4, 3 respectively and the average technique efficiency were 95.3%, 89.9%, 92.7% respectively.3) The average hospitalization expense on advanced schistosomiasis patients was 4662.69 Yuan. The results suggested that the controllable influencing factors were medicine cost, hospitalization days, and examination treatment expenses; the uncontrollable factors were complication, surgical treatment and the history of resection splenectmoy.4) The average score of EQ-5D was 0.743. The evaluation of mobility, self-care, usual activities and emotion feeling of advanced schistosomiasis patients were better. The rates of satisfaction were 69.1%, 60.0%, 60.0% and 63.6% respectively. The evaluation of body was relatively poor .The rates of satisfaction were 36.4%. Seven out of eight aspects of satisfaction degree for advanced schistosomiasis treatment institutions achieved 70.0%.Conclusions and SuggestionsConsidering the main problems and the acquired effects of the assistance work of advanced schistosomiasis, this study indicated several suggestions as follows:1) The resources utilization level of 8 designated hospitals of advanced in Jingzhou was relatively high, but it still remains a big room for efficiency improvement and it should be improved according to the quantitative results of DEA; 2)The assistance on advanced schistosomiasis is suitable, and diversiform means should be adopted to allocate limited health resources reasonably and optimally; 3) Adhere to rational drug use, so as to control drag expenses appropriately; 4) Choose the examining and treating programs reasonable as to yield the greatest value ; 5) Shorten hospitalization days as to improve medical quality; 6) intensify the strength of general survey ,guiding the patients to receive medical treatment timely and reasonably ; 7)The quality of life and satisfaction to medical institution were high, but it still remains a big room for improvement. The environments of live and production should be improved and subsequent manage work after discharge should be more perfect. |
