With the huge rapid development of heart surgery in professional hospital home and abroad in recent twenty years, the cases of complicated heart surgery and re-operation become more and more, it takes 10%-20% abroad by report. Re-operation do prolong the re-operation time of patients, increase the injury of surgery, and it do make complication after heart surgery happen.Except the lower heart function and the general condition of the patients, compared with the first heart surgery, the main cause are the pericardial adhesions, combining heart, great vessels with close and thick organization.During these years,with the test techniques and the intensive realise developing, the pericardial adhesions, as a usual complication after heart surgery, has been treated by many kinds of drug and methods. But now we still have problems or trouble:(1) It is expensive that using advanced biotic material in pericardial cavity,and this limits their use. (2) The inexpensive preventive methods now used by the traditional Chinese medicine or west medicine are not very significant, just using several drug injection. (3) The one time given methods in pericardial cavity are usual, but several times given method like Chinese medicine given by pericardial cavity perfusion through a drainage-tube is seldom, even it is a press close to clinical, inexpensive, less complication method. Here we set a rabbit experiment to study whether Xiangdan injection can provent pericardial adhesions by pericardial cavity perfusion.Objective To study the preventive effect of Xiangdan injection on pericardial adhesions,and to probe into the mechanism of it.Methods 20 Japanese flap-eared white rabbits were divided into 2 groups (n=10 each):model group and Xiangdan injection-prevented group. The test drugs were given by pericardial cavity perfusion. The model group were given sodium chloride injection, and Xiangdan injection were given to another group. The levels of fibronectin(FN) in pericardial cavity were measured and statistically analyzed during the first four days after model development. At last the pericardial adhesions were observed. We also observe whether Xiangdan injection can provent pericardial adhesions by some clinical cases after heart surgery. Results It was confirmed with pathology that the rabbit models of pericardial adhesions was successfully made, and it can be widely used for research and experiment. The levels of FN showed no significant difference after the first day (no drug were given to animals at this time). After a three day's drug given, the levels of FN showed significant difference. Traditional Chinese drug can prevent pericardial adhesions when examined by histology. The Xiangdan injection-prevented group were lighter than the model group in. pericardial adhesionsConclusion Rabbit models of pericardial adhesions was successfully made by our improved surgery. The Xiangdan injection have favorable preventive effect of pericardial adhesions,and this may because Xiangdan injection can decrease levels of FN. The introduction of traditional Chinese drug can prevent pericardial adhesions satisfactory. |
