Objective:The cerebral infarction also known as ischemic stroke. It accounts for 80%-85% of cerebrovascular disease, and the high incidence of morbidity high, seriously affecting people's health and quality of life. For decades, people in ischemic cerebrovascular disease has been a great breakthrough in research on ischemic cerebrovascular disease risk factors in the discovery and control of cerebral infarction within the time window of thrombolytic therapy and cerebral protective agents applications greatly reduce the mortality and morbidity of the disease. However, to really carry out the work of the hospital early thrombolytic and get real benefits from this treatment were very few patients. In recent years, acupuncture treatment for stroke carried out extensive and in-depth study, especially in the scalp acupuncture treatment of stroke to obtain a more satisfactory results. Scalp treatment of acute, recovery and sequelae of stroke have significant effect in stroke caused by a variety of symptoms and signs, especially hemiplegia achieved satisfactory results. Scalp of the subject of international standardization of treatment programs, through clinical and experimental research to compare the scalp acupuncture group and rehabilitation physical therapy group, neurological deficit score, clinical efficacy, motor function and activities of daily living before and after treatment and mutual assessment of the difference between; head for acute, cerebral ischemia neurological deficits, ischemic brain cyclooxygenasecox-2, nuclear factor-kappa B and transforming growth factor-beta1 content, both from clinical and experimental study of cerebral infarction hemiplegia treated with scalp acupuncture's clinical efficacy and its possible mechanism.Methods:1.Clinical study:68 patients from Wuhan Hospital of Integrated Traditional Chinese and Western medical acupuncture and neurological treatment of patients, are consistent with diagnostic criteria. According to the random number table, all the patients were randomly divided into rehabilitation therapy group and scalp acupuncture group,34 in each group, with a course of 28 days, compared rehabilitation therapy group and scalp acupuncture group of neurological deficit score, clinical efficacy, physical exercise assessment of functions and activities of daily living before and after treatment and mutual differences.2.Animal study:70 healthy SD male rats were randomly divided into sham operation group, model group and scalp acupuncture group, according to the different reperfusion times (24h,48h,72h), each divided into three subgroup, the use of intraluminal middle cerebral artery occlusion and reperfusion model. With scalp treatment, followed by Han needle acupoint nerve stimulator. Application of neurological deficit score, polymerase chain reaction and enzyme-linked immunosorbent assay reaction of acute cerebral ischemia reperfusion rat model of all time nod for neurological deficits, ischemic brain cyclooxygenasecox-2, nuclear factor-kappa B and transforming growth factor-beta1 content.Results:1.Clinical study:Rehabilitation therapy group and the scalp acupuncture group neurological function in patients after treatment compared to their pre-treatment were significantly reduced (P<0.05), and scalp acupuncture group after treatment was significantly lower than rehabilitation therapy group (P<0.05). The total effect in patients of rehabilitation therapy group and the scalp acupuncture group with significant differences (P<0.05), rehabilitation therapy group, the total effective rate was 70.59%, total effective rate of scalp acupuncture group 85.29%, indicating that the total effect of scalp acupuncture group than rehabilitation therapy group. Limb motor function score of the comparison shows that after treatment, both groups significantly increased for the treatment of homogeneous (P<0.05), while the scalp acupuncture group than rehabilitation therapy group (P<0.05). In activities of daily living score, and after treatment, both groups significantly higher than before treatment (P<0.05), and scalp acupuncture group was significantly higher than that of rehabilitation therapy group (P<0.05).2.Animal study:Scalp acupuncture group and model group, each phase of the NSS, the difference was significant (P<0.05), to 72h after cerebral ischemia and reperfusion obvious. Cyclooxygenasecox-2 and nuclear factor-kappa B content of scalp acupuncture group than in model group were lower than in 24,48,72 h model group (P<0.05), while the scalp acupuncture group transforming growth factor-beta1 content in 24,48,72 h were significantly higher than the model group (P<0.01).Conclusion:1.Scalp acupuncture therapy is conducive to the promotion of cerebral infarction patients recovery of neurological function and reduce neurological damage, and thus improve symptoms and reduce disability.2.Scalp acupuncture therapy and rehabilitation therapy are of cerebral infarction patients with hemiplegia has a therapeutic effect. It is commonly used for treatment of cerebral infarction patients. But the Internationally Standardised Scheme of Scalp Acupuncture gives prominence to traditional chinese medicine, and is easy to operate, also, featured with the standardized treatment, it is apt for international exchange, therefore is deserved widespread application.3.Clinical studies have proven that rehabilitation therapy group, the total effective rate was 70.59%, total effective rate of scalp acupuncture group 85.29%, indicating that the total effect of scalp acupuncture group than rehabilitation therapy group.In comparing patients in the recovery of limb motor function and activities of daily living, scalp treatment is also significantly better than rehabilitation therapy.4.Conducive to cerebral ischemia scalp recovery of neurological function, reduce acute cerebral neurons after ischemia-reperfusion damage and reduce the damage in certain tissues within the expression of cyclooxygenasecox-2 and nuclear factor-kappa B, and enhance the expression of and transforming growth factor-beta1, thereby slowing the immune inflammatory response, reducing cerebral ischemia-reperfusion injury.
